摘要
目的:探讨家庭医生模式干预对糖耐量减低患者骨代谢的影响。方法:以2022年1-12月在上海宜川社区卫生服务中心体检的,符合纳入和排除标准的120例糖耐量减低患者作为研究对象,将60例采用家医模式干预的患者设为干预组,将仅进行电话随访的60例患者设为对照组。观察两组患者干预前和干预后6个月的糖代谢、骨代谢相关指标的变化。结果:干预组干预前空腹血糖、糖化血红蛋白、自我效能评分、骨钙素、25羟维生素D、甲状旁腺激素、血清1型骨胶原N端肽、血清1型胶原交联C末端肽水平与对照组相比差异均无统计学意义(均P>0.05);干预后6个月后,干预组空腹血糖、糖化血红蛋白、自我效能评分、骨钙素、25羟维生素D、甲状旁腺激素、血清1型骨胶原N端肽、血清1型胶原交联C末端肽水平均优于治疗前和对照组,差异均有统计学意义(均P<0.05)。结论:家庭医生模式干预可改善糖耐量减低患者的糖代谢和骨代谢水平,值得在临床中推广应用。
Objective:To explore the effect of intervention of family doctor mode on bone metabolism in patients with impaired glucose tolerance.Methods:One hundred and twenty patients with impaired glucose tolerance who had health examination and met the admission and discharge standards at Yichuan Community Health Service Center of Shanghai from January to December 2022 were taken as the research objects,and 60 patients who were intervened with family doctor mode were set as an observation group,and 60 patients who only had telephone follow-up as a control group.The changes of glucose metabolism and bone metabolism related indexes of the two groups of patients were observed before and after 6 months of intervention.Results:Before intervention,fasting blood glucose,glycosylated hemoglobin,self-efficacy scores,osteocalcin,25 hydroxyvitamin D,parathyroid hormone,serum type 1 collagen N-terminal peptide(PINP)and serum type 1 collagen cross-linked C-terminal peptide(PICP)in the observation group had no significant differences in levels compared to the control group(P>0.05);after 6 months of intervention,fasting blood glucose,osteocalcin,self-efficacy scores,25 hydroxyvitamin D,parathyroid hormone,PINP and PICP in the observation group were significantly better than those before treatment and in the control group,and the differences were statistically significant(P<0.05).Conclusion:The intervention of family medical mode can improve the levels of glucose metabolism and bone metabolism in the patients with impaired glucose tolerance,which is worth popularizing in clinical practice.
