正畸微型种植体治疗安氏Ⅱ类错颌畸形患者牙齿美观度、口腔结构及牙周微环境的影响

The effects of orthodontic micro implants on the dental aesthetics,oral structure,and periodontal microenvironment in patients with Class Ⅱ malocclusion

摘要 目的分析正畸微型种植体对安氏Ⅱ类错颌畸形患者的影响。方法选取2022年5月—2023年5月商丘市中医院口腔科收治的84例安氏Ⅱ类错颌畸形患者作为研究对象,按照治疗方式不同分为两组,各42例。对照组行传统正畸治疗,观察组行正畸微型种植体治疗。对比两组临床疗效、牙齿美观度、咀嚼功能、口腔结构、口腔健康情况、牙周微环境、并发症。结果观察组治疗总有效率为95.24%(40/42),高于对照组的80.95%(34/42),并发症发生率为7.14%(3/42),低于对照组的23.81%(10/42);且治疗后的红色美学指数(PES)评分为(7.96±1.12)分、咬合力为(73.49±7.84)Ibs、咀嚼效率为(57.89±4.63)%,高于对照组的(6.01±0.89)分、(75.21±6.48)Ibs、(53.32±4.15)%,磨牙移位距离为(3.69±0.53)mm,小于对照组的(6.31±1.28)mm,中切牙倾角差为(25.71±3.02)°、上中切牙凸距差为(4.19±0.68)mm,大于对照组的(12.48±2.35)°、(2.41±0.35)mm,牙龈指数(GI)为(0.63±0.06)分、菌斑指数(PL)为(0.80±0.11)分、基质金属蛋白酶-2(MMP-2)为(2.28±0.36)μg/g、基质金属蛋白酶-9(MMP-9)为(2.40±0.21)μg/g,低于对照组的(0.89±0.15)分、(1.21±0.18)分、(3.67±0.53)μg/g、(3.76±0.29)μg/g(P<0.05)。结论正畸微型种植体能够恢复牙周结构,改善牙周微环境,提高牙齿美观度,有利于咀嚼功能的改善,且无严重并发症。 Objective To analyze the impact of orthodontic micro implants on patients with Class Ⅱ malocclusion.Methods A total of 84 patients with Class Ⅱ malocclusion admitted to the Stomatology Department of Shangqiu Traditional Chinese Medicine Hospital from May 2022 to May 2023 were selected as the study subjects.They were divided into two groups according to different treatment methods,with 42 cases in each group.The control group received traditional orthodontic treatment,while the observation group received orthodontic mini implant treatment.Compare the clinical effificacy,tooth aesthetics,chewing function,oral structure,oral health status,periodontal microenvironment,and complications between two groups.Results The total effective rate of the observation group was 95.24%(40/42),higher than the control group's 80.95%(34/42),and the incidence of complications was 7.14%(3/42),lower than the control group's 23.81%(10/42);After treatment,the Red Aesthetics Index(PES)score was(7.96±1.12)points,bite force was(73.49±7.84)Ibs,and chewing effificiency was(57.89±4.63)%,which was higher than the control group's(6.01±0.89)points,(75.21±6.48)Ibs,and(53.32±4.15)%.The distance of molar displacement was(3.69±0.53)mm,which was lower than the control group's(6.31±1.28)mm.The difference in inclination angle of the central incisors was(25.71±3.02)°,and the difference in protrusion distance of the upper central incisors was(4.19±0.68)mm,which was higher than that of the control group.12.48±2.35°,(2.41±0.35)mm,gingival index(GI)is(0.63±0.06),plaque index(PL)is(0.80±0.11),matrix metalloproteinase-2(MMP-2)is(2.28±0.36)μG/g,matrix metalloproteinase-9(MMP-9)is(2.40±0.21)μG/g,lower than the control group's(0.89±0.15)points,(1.21±0.18)points,and(3.67±0.53)pointsμG/g,(3.76±0.29)μG/g(P<0.05).Conclusion Orthodontic micro implants can restore periodontal structure,improve periodontal microenvironment,enhance tooth aesthetics,and facilitate the improvement of chewing function without serious complications.
