经鼻高流量氧疗对急性呼吸衰竭患者呼吸功能及预后的影响

Effects of High-Flow Nasal Cannula Oxygen Therapy on Respiratory Function and Prognosis of Patients with Acute Respiratory Failure

摘要 目的:探讨经鼻高流量氧疗对急性呼吸衰竭(ARF)患者呼吸功能及预后的影响.方法:选取2021年11月至2022年11月于本院收治的90例ARF患者作为研究对象.按照数字表法随机将其分为高流组(n=45,采取经鼻高流量氧疗)和普通组(n=45,采取经鼻导管氧疗).对两组动脉血气分析指标和呼吸功能,疾病转归和不良事件发生率进行统计学分析.结果:治疗后,两组患者动脉血氧分压(PaO_(2))、血氧饱和度(SpO_(2))均较治疗前上升,且高流组上升程度较普通组均更明显;动脉二氧化碳分压(PaCO_(2))、心率、呼吸频率(RR)、劳累度评估量表(Borg)评分和舒适度评分均较治疗前下降,且高流组下降程度较普通组均更明显,差异均有统计学意义(P<0.05).高流组ICU停留和住院时间、机械通气使用、院内感染、肺不张及不良事件总发生率均低于普通组,差异均有统计学意义(P<0.05).结论:经鼻高流量氧疗能有效改善ARF患者呼吸功能及预后情况. Objective:To investigate effects of high-flow nasal cannula oxygen therapy on respiratory function and prognosis of patients with acute respiratory failure(ARF).Methods:A total of 90 patients with ARF who were treated in the hospital from November 2021 to November 2022 were selected in this study.They were divided into a high-flow group and a general group by random number table method,with 45 cases in each group.The high-flow group was given high-flow nasal cannula oxygen therapy,while the general group was given nasal cannula oxygen therapy.indicators of the arterial blood-gas analysis as well as respiratory function,prognosis,and adverse events between two groups were statistically analyzed.Results:After treatment,both groups compared to before treatment arterial partial pressure of oxygen(PaO_(2))and oxygen saturation(SpO_(2))were all increased,and the increase were more significant in high-flow group than those of normal group,there were statistically significant differences(P<0.05).After treatment,both groups compared to before treatment arterial partial pressure of carbon dioxide(PaCO_(2)),heart rate,respiratory rate(RR),fatigue assessment scale(Borg)scores and comfort scores were all decreased.and the decrease were more significant in high-flow group than those of normal group,there were statistically significant differences(P<0.05).The duration of stay in ICU and hospitalization time,mechanical ventilation rate,the incidence of nosocomial infection and atelectasis,and the total incidence of adverse events in the high-flow group were shorter/lower than those in the general group,and the differences were statistically significant(P<0.05).Conclusion:High-flow nasal cannula oxygen therapy can effectively improve respiratory function and prognosis of patients with ARF.
