摘要
目的:观察扶阳涤痰解郁汤联合草酸艾司西酞普兰对缺血性卒中后抑郁患者抑郁症状、睡眠质量的影响,并分析卒中后抑郁症状与睡眠障碍的相关性。方法:选取缺血性卒中后抑郁患者86例,按照随机数字表法分为对照组和观察组各43例,治疗过程脱落5例,最终对照组39例、观察组42例纳入研究,对照组给予草酸艾司西酞普兰治疗;观察组在对照组基础上给予扶阳涤痰解郁汤,两组均治疗8周,分别于治疗前、治疗第4、第8周末观察两组临床疗效、抑郁症状及睡眠状况,采用汉密尔顿抑郁量表(HAMD-24)评价抑郁状态,匹兹堡睡眠质量指数量表(PSQI)评价睡眠质量,对比评估两组中医证候量表评分,并分析睡眠质量与抑郁症状之间的相关性。结果:对照组总有效率为69.23%,观察组总有效率为88.10%,观察组总有效率优于对照组(P<0.05),治疗后4周、8周观察组HAMD-24总粗分及各因子量表评分、PSQI量表评分均不同程度低于对照组(P<0.05,P<0.01),治疗后观察组中医证候因子积分不同程度低于对照组(P<0.05,P<0.01),匹兹堡睡眠质量指数量表评分与抑郁症状存在相关性。结论:扶阳涤痰解郁汤联合草酸艾司西酞普兰有效改善缺血性卒中后抑郁患者的抑郁症状,调节睡眠质量,明确抑郁症状的变化与睡眠状况存在相关性,对卒中后抑郁的临床症状有显著改善作用并降低复发的风险。
Objective To observe the effect of Fuyang Ditan Jieyu Decoction Combined with escitalopram oxalate on depression symptoms and sleep quality in patients with post-stroke depression,and analyze the correlation between post-stroke depression symptoms and sleep disorders.Methods A total of 86 patients with post-stroke depression hospitalized were selected and randomly divided into the control group and the study group,with 43 cases in each group.Five cases fell off during the treatment.Finally,39 cases in the control group and 42 cases in the study group were included in the study.The control group was treated with escitalopram oxalate,and the study group was treated with Fuyang Ditan Jieyu Decoction on the basis of the control group.Both groups were treated for 8 weeks At the end of the 4th and 8th weeks of treatment,the clinical efficacy,depressive symptoms and sleep status of the two groups were observed.Hamilton Depression Scale(hamd-24)was used to evaluate the depression status,Pittsburgh sleep quality index(PSQI)was used to evaluate the sleep quality,and the TCM Symptom Scale scores of the two groups were compared and evaluated,and the correlation between sleep quality and depressive symptoms was analyzed.Results The total effective rate of the control group was 88.10%,and the study group was 69.23%.The total effective rate of the study group was better than the control group(P<0.05);At 4 and 8 weeks after treatment,the total crude score of hamd-24,the score of each factor scale,and the score of PSQI scale in the study group were lower than the control group(P<0.05,P<0.01).After treatment,the TCM syndrome factor score of the study group was lower than the control group(P<0.05,P<0.01),There was a correlation between Pittsburgh sleep quality index score and depressive symptoms.Conclusion Fuyang Ditan Jieyu Decoction Combined with escitalopram oxalate can effectively improve the depressive symptoms of patients with post-stroke depression,regulate the quality of sleep,clarify the correlation between the changes of
