摘要
心房颤动(房颤)是一种临床常见的心律失常,随着年龄的增长,其发病率呈现显著升高趋势。房颤可导致血栓形成、脑卒中、肾功能不全等严重并发症,极大危害老年人的身心健康,降低其生活质量,并给社会造成了严重的经济负担。近年来,房颤的全程综合管理成为研究热点,但其规范化管理路径尚未形成,家庭医生团队模式作为一种全程闭环管理服务模式,可为老年房颤患者提供全面、连续的医疗服务新路径,本文对老年房颤的综合管理发展进行简介,并结合外冈镇社区卫生服务中心对老年房颤患者实施家庭医生团队管理路径实践中取得的成效,对怎样开展同质化、规范化房颤管理路径进行探讨及思考。
Atrial fibrillation(AF) is a common arrhythmia in clinical practice.With the increase of age,its incidence rate shows a significant upward trend.AF can lead to serious complications such as thrombosis,stroke,and renal insufficiency,greatly endangering the physical and mental health of the elderly,reducing their quality of life,and causing serious economic burden to society.In recent years,the comprehensive management of AF has become a research hotspot,but its standardized management path has not yet been formed.The family doctor team model,as a closedloop management service model,can provide a comprehensive and continuous medical service new path for elderly AF patients.This article briefly introduces the development of comprehensive management of elderly AF,and combines the achievements of the Waigang Town Community Health Service Center in implementing the family doctor team management path for elderly AF patients to explore and think about how to carry out a homogeneous and standardized AF management path.
