初诊2型糖尿病老年患者血糖变异性与肠道菌群的关系

摘要 目的探讨初诊2型糖尿病(T2DM)老年患者血糖变异性(GV)相关的肠道菌群特征。方法纳入2023年2月至2023年9月北京医院内分泌门诊治疗的40例初诊老年T2DM患者,按照平均血糖波动幅度(MAGE)及变异系数(CV)进行分组,持续葡萄糖监测并记录监测期间的饮食情况,分析不同GV水平的肠道菌群差异。结果2种分组结果均提示初诊T2DM老年患者的GV较大,高MAGE比例为70%,高CV为25%。采用对应糖化血红蛋白范围的CV界值较MAGE更有利于控制变量,以评估不同血糖控制水平的血糖变异性。纺锤链杆属、粪杆菌属与2种GV指标呈正相关;在MAGE分组研究中挑剔真杆菌属、瘤胃球菌科、梭菌纲与MAGE水平呈正相关。结论初诊T2DM老年患者在疾病初期可见较大的血糖波动,肠道纺锤链杆菌属、粪杆菌属、挑剔真杆菌属、瘤胃球菌科可能影响T2DM患者血糖波动。 Objective To explore the gut microbiota characteristics related to glucose variability(GV)in newly diagnosed elderly patients with type 2 diabetes(T2DM).Methods Forty newly diagnosed elderly patients with T2DM were included in the endocrinology outpatient department of Beijing Hospital from February 2023 to September 2023.They were grouped according to mean amplitude of glycemic excursions(MAGE)and coefficient of variation(CV),continuous glucose monitoring and recording of dietary habits during the monitoring period,analyzing differences in gut microbiota at different GV levels.Results Both grouping results indicated significant blood glucose variability in elderly newly diagnosed T2DM patients,with a high MAGE ratio of 70%and a high CV of 25%.Using CV thresholds corresponding to the HbA1c range was more advantageous in controlling variables and evaluating blood glucose variability at different levels of blood glucose control compared to MAGE.There was a positive correlation between spindle chain rod genus and fecal bacilli genus and two types of GV indicators;In the MAGE grouping study,there was a positive correlation between the levels of MAGE and the genera of Bacteroides,Ruminococcaceae,and Clostridia.Conclusions Elderly patients with newly diagnosed T2DM may exhibit significant glycemic fluctuations in the early stages of the disease.Genera such as Fusicatenibacteria,Faecalibacterium,Eubacterium_eligens_group,and Ruminococcaceae may influence glycemic fluctuations in elderly patients with T2DM.
