摘要
目的:探究积雪苷霜软膏联合点阵CO_(2)激光对面部烧伤后增生性瘢痕(Hypertrophic scar,HS)的临床疗效。方法:选取2020年7月-2022年7月宿迁市第一人民医院收治的80例面部烧伤后HS患者为研究对象,按照随机数字表法将其分为对照组(n=40)和观察组(n=40),对照组给予点阵CO_(2)激光治疗,观察组在对照组基础上联合积雪苷霜软膏治疗,连续治疗6个月,比较两组患者临床疗效,以及治疗前后改良温哥华瘢痕评估量表(Modified Vancouver scar scale,mVSS)、瘢痕血流灌注量、瘢痕厚度和烧伤健康量表(Burns specific health scale-brief,BSHS-B)评分,记录不良反应发生情况。结果:观察组临床总有效率(92.50%)较对照组(72.50%)高(P<0.05);治疗前,两组患者色泽、厚度、柔软度、血管分布、瘙痒、疼痛及mVSS总分比较差异无统计学意义(P>0.05),治疗后,两组患者上述评分均降低,且观察组较对照组更低(P<0.05);治疗前,两组患者瘢痕血流灌注量、瘢痕厚度及BSHS-B各维度评分比较差异无统计学意义(P>0.05),治疗后,两组患者瘢痕血流灌注量、瘢痕厚度均降低,BSHS-B各维度评分均升高,且观察组优于对照组(P<0.05);两组患者不良反应总发生率对比差异无统计学意义(P>0.05)。结论:面部烧伤后HS患者应用积雪苷霜软膏联合点阵CO_(2)激光治疗,疗效显著,可明显改善mVSS评分、瘢痕血流灌注量及瘢痕厚度,提高生存质量,且安全性良好。
Objective To investigate the clinical efficacy of asiaticoside ointment combined with fractional carbon dioxide(CO_(2))laser in the treatment of hypertrophic scar(HS)after facial burn.Methods A study from July 2020 to July 2022 were conducted among 80 patients with HS after facial burn who were admitted to Suqian First People's Hospital.The patients were assigned to the control group(n=40)and the observation group(n=40)by random number table method.Patients in the control group were treated with fractional CO_(2) laser.Patients in the observation group were treated with asiaticoside ointment combined with fractional CO_(2) laser.All patients were treated for 6 months.The two groups were compared in terms of clinical efficacy,the modified Vancouver Scar Scale(mVSS)score,blood perfusion and thickness of the scar,and the Burn Specific Health Scale-Brief(BSHS-B)score.Adverse reactions were recorded.Results The total clinical response rate in the observation group(92.50%)was significantly higher as compared with that in the control group(72.50%)(P<0.05).After treatment,the scores for color,thickness,softness,distribution of blood vessels,pruritus and pain and the total scores of mVSS in the two groups decreased.The scores of observation group were lower as compared with those of the control group(P<0.05).Blood perfusion and thickness of scars decreased,and the BSHS-B scores increased in the two groups.The above indicators in observation group were better than those in the control group(P<0.05).The total incidence rates of adverse reactions in the two groups were close(P>0.05).Conclusion Asiaticoside ointment combined with fractional CO_(2) laser is effective in the treatment of HS after facial burn.The combined treatment can significantly improve the mVSS score,blood perfusion and thickness of the scar,and quality of life,with good safety.
