摘要
目的:探讨早期功能锻炼对桡骨远端骨折手法复位后动态小夹板固定患者肢体肿胀、疼痛及功能恢复的影响。方法:选取2022年3月—2023年3月四川省骨科医院收治的94例桡骨远端骨折患者,根据随机数字表法分为试验组和对照组,各47例。对照组行手法复位、动态小夹板固定治疗及常规康复训练,试验组在对照组基础上予以早期功能锻炼。比较两组患者治疗前后肿胀值、疼痛度、腕关节功能、炎症因子水平、临床疗效及骨痂愈合情况。结果:治疗后,两组患者肿胀值、视觉模拟量表疼痛评分均低于治疗前,且试验组明显低于对照组(P<0.05)。两组患者Cooney腕关节评分组间、时间及交互比较,差异均有统计学意义(P<0.05);两组患者治疗后不同时点Cooney腕关节评分均高于治疗前,且试验组显著高于对照组,差异均有统计学意义(P<0.05)。两组患者治疗后C反应蛋白、白细胞介素-6水平均低于治疗前,且试验组明显低于对照组(P<0.05)。试验组总有效率为93.62%,对照组为76.60%,试验组显著优于对照组(P<0.05)。试验组Lane-Sandhu X线片评分大于对照组(P<0.05)。结论:桡骨远端骨折手法复位后动态小夹板固定患者行早期功能锻炼,可有效缓解患肢肿胀及疼痛,减轻炎症反应,促进腕关节功能恢复及骨痂愈合。
Objective To investigate the effect of early functional exercise on limb swelling,pain and functional recovery in patients with distal radius fractures undergoing dynamic small splint fixation after manual reduction.Methods A total of 94 patients with distal radius fractures who were treated in Sichuan Province Orthopedic Hospital from March 2022 to March 2023 were selected and randomly divided into treatment group and control group,with 47 patients in each group.The control group was given manual reduction,dynamic small splint fixation and routine rehabilitation training.On this basis,the treatment group was given early functional exercise.The swelling value,pain level,wrist function,the levels of inflammatory factors,clinical efficacy and callus healing were compared between the groups.Results After treatment,swelling values and Visual Analogue Scale scores of both groups decreased.The value and score of the treatment group were significantly lower(P<0.05).There were statistically significant differences in Cooney wrist joint score in terms of time effect,inter-group effect and interaction effect(P<0.05).At different time points after treatment,Cooney wrist joint scores of both groups were higher than those before treatment,and the treatment group had significantly higher scores than the control group(P<0.05).After treatment,the levels of C-reactive proteinand interleukin-6 in both groups decreased,and the levels in treatment group were significantly lower(P<0.05).The total effective rate in the treatment group(93.62%)was significantly higher,compared with the control group(76.60%).The Lane-Sandhu X-ray score in the treatment group was higher than the control group(P<0.05).Conclusion Early functional exercise can effectively alleviate limb swelling and pain,relieve inflammatory response,promote the recovery of wrist function and callus healing in patients with distal radius fractures undergoing dynamic small splint fixation after manual reduction.
