摘要
目的分析妊娠糖尿病患者实施胰岛素结合个体化营养治疗对其免疫指标以及妊娠结局改善的效果。方法选取2022年4月—2023年6月莆田学院附属医院收治的78例妊娠糖尿病患者为研究对象,通过Excel表对患者进行随机排序,并按序号的奇偶数分为对照组(常规治疗)和观察组(胰岛素联合个体化营养治疗),各39例。对比两组营养水平、胰岛指标、血糖指标、妊娠结局。结果治疗前,两组血糖水平以及糖化血红蛋白水平对比,差异无统计学意义(P均>0.05);治疗后,观察组上述指标水平均较对照组低,差异有统计学意义(P均<0.05)。治疗后,观察组血红蛋白、白蛋白、转铁蛋白水平高于对照组,差异有统计学意义(P均<0.05)。治疗后,观察组胰岛素抵抗指数低于对照组,胰岛β细胞功能指数高于对照组,差异有统计学意义(P均<0.05)。观察组不良妊娠结局发生率低于对照组,差异有统计学意义(P<0.05)。结论妊娠糖尿病患者接受胰岛素、个体化营养联合治疗,可改善患者血糖、营养水平和胰岛功能,改善妊娠结局。
Objective To analyze the effect of insulin combined with individualized nutrition therapy on immune index and pregnancy outcome in patients with gestational diabetes.Methods A total of 78 patients with gestational diabetes mellitus from April 2022 to June 2023 admitted to Putian University Affiliated Hospital were selected as the study objects.The patients were randomly sorted by Excel table,and were diviede into the control group(routine treatment)and the observation group(insulin combined with individualized nutrition treatment)according to the the odd and even number of serial numbers with 39 cases in each group.The nutritional level,islet index,blood glucose index and pregnancy outcome of two groups were analyzed.Results Before treatment,there were no significant differences in blood glucose level and glycosylated hemoglobin A1c level between the two groups(both P>0.05).After treatment,the indexes above in the observation group were lower than those in the control group,the differences were statistically significant(both P<0.05).Hemoglobin,albumin and transferrin level of the observation group were higher than those of the control group after treatment,the differences were statistically significant(all P<0.05).After treatment,the insulin resistance index of observation group was lower than that of control group,and the function index of isletβcell was higher in the observation group than that of control group,the differences were statistically significant(both P<0.05).The incidence rate of adverse pregnancy outcome in observation group was lower than that in control group,the difference was statistically significant(P<0.05).Conclusion The combination treatment of insulin and individualized nutrition for gestational diabetes patients can improve the level of blood glucsoe,nutrition and islet function,and improve the pregnancy outcome.
