摘要
目的分析早中期三阴性乳腺癌患者改良根治术后预后的影响因素。方法采用病例对照研究,选取医院2020年1月~2022年1月收治的40例改良根治术后发生复发转移的早中期三阴性乳腺癌患者临床病历资料纳入发生组,另收集医院同期40例未发生复发转移的早中期三阴性乳腺癌患者临床病历资料纳入未发生组。对比两组肿瘤直径大小、术中切缘宽度、疾病类型、淋巴结转移等资料,探寻影响早中期三阴性乳腺癌患者改良根治术后临床预后情况的因素。结果发生组肿瘤直径大小、MFSI-SF评分、淋巴结转移占比均大于未发生组,术后接受免疫治疗占比低于未发生组(P<0.05)。经Logistic回归分析结果显示,肿瘤直径大、术后免疫治疗、有淋巴结转移、MFSI-SF评分高是早中期三阴性乳腺癌患者改良根治术后复发转移发生的影响因素(P<0.05)。结论早中期三阴性乳腺癌患者经改良根治术后有一定复发转移风险,肿瘤直径大、术后免疫治疗、有淋巴结转移、癌因性疲乏程度高是影响其发生的因素。
Objective To analyze the prognostic factors of early and middle stage triple negative breast cancer patients after modified radical mastectomy.Methods A case-control study was conducted to select 40 cases of early and middle stage triple negative breast cancer patients with recurrence and metastasis after modified radical surgery in the hospital from January 2020 to January 2022 into the occurrence group,and 40 cases of early and middle stage triple negative breast cancer patients with no recurrence and metastasis in the hospital during the same period were collected into the non occurrence group.The tumor diameter,surgical margin width,disease type,lymph node metastasis and other data of the two groups were compared to explore the factors affecting the clinical prognosis of early and middle stage triple negative breast cancer patients after modified radical mastectomy.Results The tumor diameter,MFSI-SF score,and proportion of lymph node metastasis in the occurrence group were greater than those in the non occurrence group,and the proportion of postoperative immunotherapy was lower than that in the non occurrence group(P<0.05).Logistic regression analysis showed that large tumor diameter,postoperative immunotherapy,lymph node metastasis,and high MFSI-SF score were the influencing factors of recurrence and metastasis in early and middle stage triple negative breast cancer patients after modified radical mastectomy(P<0.05).Conclusion Patients with early and middle stage triple negative breast cancer have a certain risk of recurrence and metastasis after modified radical mastectomy.Large tumor diameter,postoperative immunotherapy,lymph node metastasis,and high cancer-related fatigue are the factors affecting its occurrence.
