摘要
目的:观察高分辨率磁共振血管壁成像技术(HR MR-VWI)对缺血性脑卒中(CIS)患者的评估价值。方法:选取南京医科大学第二附属医院2021年1月—2023年8月收治的120例疑似CIS患者为研究对象,患者均接受颈部血管超声(CVUS)、HR MR-VWI检查,将数字减影血管造影(DSA)诊断结果作为金标准,对比CVUS、HR MR-VWI诊断价值,同时对CIS患者责任斑块与非责任斑块HR MR-VWI特征进行比较。结果:120例疑似CIS患者经DSA诊断确诊CIS患者(阳性)80例、非CIS患者(阴性)40例,CVUS检查检出阳性65例、阴性55例,HR MR-VWI检查检出阳性74例、阴性46例。HR MR-VWI诊断准确度、特异度、灵敏度均高于CVUS,差异有统计学意义(P<0.05)。DSA确诊80例CIS患者,共计斑块320处,包括责任斑块112处、非责任斑块208处,责任斑块与非责任斑块HR MR-VWI特征比较,管腔狭窄、斑块位置、斑块形态、斑块强化差异有统计学意义(P<0.05)。结论:CIS患者诊断中应用HR MR-VWI检查,相比CVUS有更好的诊断价值,诊断效能更高,且可对颅内动脉斑块强化程度进行分析,从而评估疾病危险因素,为临床诊治提供更多参考依据,值得应用。
Objective:To evaluate the value of high resolution magnetic resonance vascular wall imaging(HR MR-VWI)in pa⁃tients with ischemic stroke(CIS).Methods:A total of one hundred and twenty suspected CIS patients admitted to the Second Affili⁃ated Hospital of Nanjing Medical University from January 2021 to August 2023 were selected as the study objects.All patients un⁃derwent cervical vascular ultrasound(CVUS)and HR MR-VWI examination.Digital subtraction angiography(DSA)was used as the gold standard to compare the diagnostic value of CVUS and HR MR-VWI.The characteristics of responsible plaque and nonresponsible plaque HR MR-VWI in CIS patients were compared.Results:Among the 120 suspected CIS patients,80 were con⁃firmed CIS patients(positive)and 40 were confirmed non-CIS patients(negative)by DSA diagnosis,65 were positive and 55 were negative by CVUS examination,and 74 were positive and 46 were negative by HR MR-VWI examination;the diagnostic accuracy and specificity sensitivity of HR MR-VWI were higher than those of CVUS,and the difference was statistically significant(P<0.05);there were 80 CIS patients diagnosed by DSA,with a total of 320 plaques,including 112 responsible plaques and 208 non-responsible plaques,compared with the HR MR-VWI characteristics of responsible plaques and non-responsible plaques,there were statistically significant differences in lumen stenosis,plaque location,plaque morphology and plaque enhancement(P<0.05).Conclusion:Compared with CVUS,the application of HR MR-VWI in the diagnosis of CIS patients has better diagnostic value and higher diagnostic efficiency,and can analyze the degree of intracranial arterial plaque enhancement,so as to evaluate the risk factors of the disease and provide more references for clinical diagnosis and treatment,which is worthy of application.
