摘要
目的:分析阿司匹林联合硝苯地平、拉贝洛尔治疗妊娠高血压的效果。方法:选取2022年1月—2023年5月湛江市妇幼保健计划生育服务中心收治的妊娠高血压患者100例为研究对象,随机分为对照组与观察组,各50例。对照组给予硝苯地平和拉贝洛尔治疗,观察组在对照组基础上联合阿司匹林治疗。比较两组治疗效果、血管内皮功能、胎盘血流状态、不良妊娠结局发生情况。结果:观察组治疗总有效率高于对照组,差异有统计学意义(P=0.012)。治疗后,两组内皮素-1、血管内皮生长因子、一氧化氮水平下降,观察组低于对照组,差异有统计学意义(P<0.05)。治疗后,两组爱帕琳肽水平升高,观察组高于对照组,差异有统计学意义(P<0.05)。治疗后,两组血液流速升高,观察组高于对照组,差异有统计学意义(P<0.05)。治疗后,两组搏动指数、阻力指数、收缩峰值流速/舒张末期最大速度水平下降,观察组低于对照组,差异有统计学意义(P<0.05)。观察组不良妊娠结局总发生率低于对照组,差异有统计学意义(P=0.029)。结论:阿司匹林联合硝苯地平、拉贝洛尔治疗妊娠高血压的效果显著,可改善血管内皮功能,恢复胎盘血流状态,降低不良妊娠结局发生率。
Objective:To analyze the effect of aspirin combined with nifedipine and labetalol in the treatment of hypertensive pregnancy.Methods:A total of 100 patients with hypertensive pregnancy were selected as the study objects at Zhanjiang Maternal and Child Health Care family Planning Service Center from January 2022 to May 2023,and were randomly divided into control group and observation group,with 50 cases in each group.The control group was treated with nifedipine and labetalol,and the observation group was treated with aspirin on the basis of control group.The therapeutic effect,vascular endothelial function,placental blood flow status and adverse pregnancy outcome were compared between the two groups.Results:The total effective rate of the observation group was higher than that of the control group,the difference was statistically significant(P=0.012).After treatment,the levels of endothelin-1,vascular endothelial growth factor and nitric oxide in two groups were decreased,and the observation group was lower than the control group,the difference was statistically significant(P<0.05).After treatment,the level of epalintide in two groups increased,and the observation group was higher than the control group,the difference was statistically significant(P<0.05).After treatment,the blood flow velocity of the two groups increased,and the observation group was higher than the control group,the difference was statistically significant(P<0.05).After treatment,the pulsation index,resistance index,peak systolic flow rate/end-diastolic maximum velocity decreased in two groups,and the observation group was lower than the control group,the difference was statistically significant(P<0.05).The incidence of adverse pregnancy outcomes in the observation group was lower than that in the control group,and the difference was statistically significant(P=0.029).Conclusion:Aspirin combined with nifedipine and labetalol can improve vascular endothelial function,restore placental blood flow and reduce the incidence of adverse pregnancy outc
