摘要
目的 观察内镜下胶圈套扎术在治疗内痔疼痛、出血、脱出等症状上的的疗效及安全性。方法 收集自2021年1月至2022年5月在福州市第二总医院行内镜下胶圈套扎术的50例患者的临床资料,分析手术相关的各项指标(内痔分期、手术时长、术前症状),术前及术后1个月、术后12个月回访的各项评分指标[包括疼痛视觉模拟量表(VAS)评分、出血频率评分、生活质量评分和脱出程度评分]及手术疗效、术后并发症、住院时间等指标,评价其治疗的有效性及安全性。结果 内镜下胶圈套扎术治疗的患者,平均手术时长(含内镜下息肉切除术)为(27.58±8.94)min,平均住院时间为(3.48±2.07)d。其中,47例患者治疗有效率为94.00%。仅3例(6.00%)患者出现术后并发症,且对症处理后症状均得以控制。术后随访12个月后发现,术后1个月、术后12个月的VAS评分、出血频率评分、生活质量评分和脱出程度评分较术前均降低。结论 内镜下胶圈套扎术用于内痔的治疗安全、有效,可缓解Ⅰ、Ⅱ、Ⅲ度内痔引起的出血、脱出、疼痛症状,能减少术后并发症。
Objective To observe the efficacy and safety of endoscopic rubber band ligation in the treatment of symptoms such as pain,bleeding,and prolapse in internal hemorrhoids.Methods Clinical data of 50 patients who underwent endoscopic rubber band ligation at Fuzhou Second General Hospital from January 2021 to May 2022 were collected.Various surgical indicators(hemorrhoid staging,duration of surgery,preoperative symptoms)were analyzed.The efficacy and safety of the treatment were evaluated by the scores of preoperative,postoperative 1 month and postoperative 12 months[including visual analogue scale of pain(VAS)score,bleeding frequency score,quality of life score and exhalation degree score],surgical efficacy,postoperative complications,length of stay and other indicators.Results Patients treated with endoscopic rubber band ligation had an average surgical duration(including endoscopic polypectomy)of(27.58±8.94)minutes and an average hospital stay of(3.48±2.07)days.Among them,47 patients had effective treatment,with a treatment effectiveness rate of 94%.Only 3 patients experienced postoperative complications,which were effectively managed.Follow-up at 12 months postoperatively revealed significant reductions in VAS score,bleeding frequency score,quality of life score,and degree of prolapse score at 1 month and 12 months postoperatively compared to pre-surgery.Conclusions Endoscopic rubber band ligation is a safe and effective treatment for internal hemorrhoids,relieving bleeding,prolapse,and pain symptoms caused by Grade I,II,III internal hemorrhoids,as well as bleeding symptoms caused by Grade IV internal hemorrhoids,with reduce complications.
