摘要
目的评估氨甲环酸联合缩宫素在减少剖宫产术后出血量方面的效果。方法选择行剖宫产术的96例产妇为研究对象,随机分为对照组及研究组,每组48例。产后,对照组使用缩宫素,研究组则在对照组基础上使用氨甲环酸。比较两组产妇术后不同时间段的出血量、凝血指标及不良反应发生率。结果研究组术后1、24h出血量分别为(226.92±51.65)ml、(319.54±73.82)ml,均低于对照组的(336.37±54.79)ml、(437.86±78.61)ml(P<0.05)。两组术后24h的凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)、纤维蛋白原(FIB)水平均改善,且研究组PT、APTT水平均低于对照组,而FIB水平高于对照组(P<0.05)。两组不良反应总发生率比较,差异无统计学意义(P>0.05)。结论氨甲环酸联合缩宫素在剖宫产后的应用可有效减少产妇出血量,改善其凝血指标,提升FIB水平,且不会提高不良反应发生率,显示出良好的临床应用价值。
Objective To evaluate the effect of tranexamic acid combined with oxytocin on reducing blood loss after cesarean section.Methods 96 parturients who underwent cesarean section were selected as the research objects and were randomly divided into a control group and a study group,with 48 cases in each group.After delivery,the control group received oxytocin,and the study group received tranexamic acid on the basis of the control group.The blood loss,coagulation indexes and incidence of adverse reactions were compared between the two groups in different time periods.Results The blood loss at 1 and 24 h after surgery in the study group were(226.92±51.65)ml and(319.54±73.82)ml,respectively,which were lower than(336.37±54.79)ml and(437.86±78.61)ml in the control group(P<0.05).The levels of prothrombin time(PT),activated partial thromboplastin time(APTT)and fibrinogen(FIB)in both groups were improved 24 h after surgery,and the levels of PT and APTT in the study group were lower than those in the control group,while the level of FIB was higher than that in the control group(P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion The application of tranexamic acid combined with oxytocin after cesarean section can effectively reduce the amount of maternal blood loss,improve the coagulation indexes,increase the FIB level,and does not increase the incidence of adverse reactions,showing good clinical application value.
