^(18)F-Florbetapir PET定量标准化Centiloid及其在中国临床前期阿尔茨海默病队列中的应用 被引量:1

Standardization of amyloid quantitation with ^(18)F-Florbetapir SUV ratio to the Centiloid scale and its application in Chinese Preclinical Alzheimer′s Disease Study

摘要 目的建立^(18)F-氟贝他吡(Florbetapir)PET图像的SUV比值(SUVR)标准化Centiloid(CL)的公式,并研究中国临床前期阿尔茨海默病(AD)(C-PAS)队列的β-淀粉样蛋白(Aβ)沉积情况。方法利用统计参数图(SPM)12对"Standard PIB"和"Florbetapir Calibration"公开数据库中的11C-匹兹堡化合物B(PIB)和^(18)F-Florbetapir图像进行预处理,得到SUVR标准化为CL的变换公式。回顾性纳入C-PAS队列于2018年10月至2023年8月在复旦大学附属华山医院核医学/PET中心接受^(18)F-Florbetapir PET检查的受试者942例[男357例、女585例,年龄(66.4±8.1)岁],计算其CL值并分析AD、轻度认知障碍(MCI)和认知未受损(CU)组的Aβ阳性率[CL值≤12为Aβ阴性,12<CL值<30为Aβ弱阳性,CL值≥30为Aβ阳性]。使用Kruskal-Wallis秩和检验、Dunn′s检验(Bonferroni法校正)和χ^(2)检验对数据进行统计分析。结果^(18)F-Florbetapir PET图像的SUVR标准化CL的计算公式为CL=179.64×SUVR_Florbetapir-186.95。在C-PAS队列中,AD组的SUVR、CL值和Aβ阳性率(含弱阳性)分别为1.29±0.22、43.97±39.23和71.80%(140/195);MCI组的SUVR、CL值和Aβ(含弱阳性)阳性率分别为1.04(1.02,1.14)、1.16(-4.04,17.14)和28.50%(61/214);CU组的SUVR、CL值和阳性率分别为1.04(1.01,1.08)、0.54(-5.29,7.69)和15.38%(82/533);3组的SUVR、CL值和Aβ沉积阴性/弱阳性/阳性比例的差异均有统计学意义(H=148.30,H=148.30,χ^(2)=262.12,均P<0.001)。此外,混合型MCI较非遗忘型MCI表现出更高的CL值[2.45(-1.54,46.32)与-1.58(-6.33,7.20);H=8.21,P=0.016;z=2.81,P=0.015]和Aβ阳性率(含弱阳性)[41.18%(14/34)与14.64%(6/41);χ^(2)值:12.71和10.63,均P<0.01];CU组的CL值和Aβ阳性率随年龄增长而增加。结论本研究验证了^(18)F-Florbetapir PET图像计算CL值的可行性,并揭示了C-PAS队列中Aβ沉积情况,为国内开展多中心AβPET研究奠定了基础。 Objective To standardize the quantitation of ^(18)F-Florbetapir PET SUV ratio(SUVR)to the Centiloid(CL)scale,and analyze the positive rate of β-amyloid(Aβ) in Chinese Preclinical Alzheimer′s Disease(AD)Study(C-PAS).Methods 11C-Pittsburgh compound B(PIB)and ^(18)F-Florbetapir images from public databases"Standard PIB"and"Florbetapir Calibration"were preprocessed by statistical parametric mapping(SPM)12,and the transformative formulas from SUVR to CL were derived.Then a total of 942 subjects(357 males,585 females;age(66.4±8.1)years)from C-PAS who received^(18)F-Florbetapir PET at the Department of Nuclear Medicine&PET Center,Huashan Hospital,Fudan University from October 2018 to August 2023 were retrospectively included.CL values were calculated and the Aβpositive rates(CL value≤12,Aβnegative;12<CL value<30,Aβsubtle pathology;CL value≥30,Aβpositive)of AD,mild cognitive impairment(MCI)and cognitive unimpaired(CU)groups were explored.Data were analyzed by using Kruskal-Wallis rank sum test,Dunn′s test(Bonferroni correction)andχ^(2)test.Results The formula for the ^(18)F-Florbetapir SUVR converted to CL was CL=179.64×SUVR_Florbetapir-186.95.In the C-PAS cohort,the SUVR,CL value,Aβpositive rate(including subtle pathology)of patients with clinically diagnosed AD were 1.29±0.22,43.97±39.23,71.80%(140/195),which were 1.04(1.02,1.14),1.16(-4.04,17.14),28.50%(61/214)for patients with MCI,and 1.04(1.01,1.08),0.54(-5.29,7.69),15.38%(82/533)for CU subjects,respectively.SUVR,CL value and the ratio of negative,subtle and positive Aβpathology of the above three groups exhibited statistical differences(H=148.30,H=148.30,χ^(2)=262.12,all P<0.001).Besides,mixed MCI group exhibited higher CL values((2.45(-1.54,46.32)vs-1.58(-6.33,7.20);H=8.21,P=0.016;z=2.81,P=0.015)and Aβpositive rate(including subtle pathology)(41.18%(14/34)vs 14.64%(6/41);χ^(2)values:12.71 and 10.63,both P<0.01),compared to non-amnestic MCI group.The CL values and Aβpositive rates were also increased with age in CU group.Conclusion This stu
