摘要
目的观察复方丹参滴丸联合雷米普利片对糖尿病肾病(DN)患者肾功能、P选择素水平及糖脂代谢的影响。方法将88例DN患者按照随机数字表法分为2组,对照组44例予雷米普利片治疗,治疗组44例在对照组治疗基础上予复方丹参滴丸治疗。2组均治疗4周。比较2组治疗前后中医证候评分、肾功能[血尿素氮(BUN)、血肌酐(SCr)及尿蛋白排泄率]、糖脂代谢[餐后2 h血糖(2 hPG)、空腹血糖(FPG)、甘油三酯(TG)、总胆固醇(TC)]、炎症因子[超敏C反应蛋白(hs-CRP)、P选择素、肿瘤坏死因子α(TNF-α)]水平及不良反应发生率。结果2组治疗后中医证候各项评分均较本组治疗前降低(P<0.05),且治疗组治疗后均低于对照组(P<0.05)。2组治疗后BUN、SCr、尿蛋白排泄率均较本组治疗前降低(P<0.05),且治疗组治疗后均低于对照组(P<0.05)。2组治疗后FPG、2 hPG、TG、TC水平均较本组治疗前降低(P<0.05),且治疗组治疗后均低于对照组(P<0.05)。2组治疗后血清hs-CRP、P选择素、TNF-α水平均较本组治疗前降低(P<0.05),且治疗组治疗后均低于对照组(P<0.05)。治疗组不良反应发生率11.36%(5/44),对照组不良反应发生率4.55%(2/44),2组比较差异无统计学意义(P>0.05)。结论复方丹参滴丸联合雷米普利片治疗DN可有效提高肾功能,改善糖脂代谢,减轻炎症反应,降低P选择素水平,安全有效。
Objective To explore the effects of compound Danshen dripping pills(CDDP)combined with ramipril tablets on renal function,soluble P-selectin(sP-sel)and glucose and lipid metabolism in patients with diabetic nephropathy(DN).Methods A total of 88 DN patients were randomly assigned into treatment group(n=44)and control group(n=44).All patients were managed by ramipril tablets,and those in treatment group were additionally treated with CDDP.A 4-week treatment was performed,aiming to compare traditional Chinese medicine(TCM)syndrome score,renal function(blood urea nitrogen[BUN],serum creatinine[SCr],urine protein excretion rate),glucose and lipid metabolism(2-h postprandial plasma glucose[2-hPG],fasting plasma glucose[FPG],triglycerides[TG],total cholesterol[TC]),inflammatory factors(high-sensitivity C-reactive protein[hs-CRP],sP-sel,tumor necrosis factorα[TNF-α]),incidence of adverse reactions.Results After treatment,TCM symptom scores in the both groups were significantly decreased(P<0.05),which decreased notably in the treatment group compared with the control group(P<0.05).BUN,SCr and urine protein excretion rate in the both groups were significantly decreased(P<0.05),the decrease in the treatment group was more common than the control group(P<0.05).FPG,2 hPG,TG,TC in the both groups were significantly decreased(P<0.05),the treatment group was superior to the control group for FPG,2 hPG,TG,TC(all P<0.05).The hs-CRP,sP-sel and TNF-αin the both groups were significantly decreased(P<0.05),the decrease in the treatment group was more prevalent compared with the control group(P<0.05).The difference was not statistically significant in the incidence of adverse reactions between the treatment group and the control group(11.36%[5/44]vs 4.55%[2/44],P>0.05).Conclusion For DN patients,CDDP combined with ramipril tablets can effectively improve renal function,improve glucose and lipid metabolism,reduce inflammatory response and sP-sel levels,and with safety and effectiveness.
