摘要
目的:探讨社区高血压慢性病管理模式在老年高血压患者中的应用效果。方法:选取2020年1月—2021年12月北京市西城区金融街社区卫生服务中心收治的80例老年高血压患者作为研究对象,随机分为对照组与研究组,各40例。对照组开展常规管理模式,研究组开展社区高血压慢性病管理模式。比较两组管理效果。结果:管理后,两组收缩压、舒张压低于管理前,且研究组低于对照组,两组血压达标率高于管理前,且研究组高于对照组,差异有统计学意义(P<0.05)。日间及夜间,研究组收缩压波动值、舒张压波动值低于对照组,差异有统计学意义(P<0.001)。管理后,两组血压监测管理、用药管理、饮食管理、运动管理评分高于管理前,且研究组高于对照组,差异有统计学意义(P<0.05)。研究组对管理实用性、可操作性、便利性、持续改进性的满意评分高于对照组,差异有统计学意义(P<0.001)。结论:社区高血压慢性病管理模式在老年高血压患者中的应用效果显著,能够改善患者血压,减少血压波动,提高患者血压达标率、自我管理能力、管理模式满意度。
Objective:To discuss the application effect of community hypertension chronic disease management mode in elderly patients with hypertension.Methods:A total of 80 elderly hypertension patients admitted to Beijing Xicheng District Financial Street Community Health Service Center from January 2020 to December 2021 were selected as the study subjects.They were randomly divided into control group and study group,with 40 cases in each group.The control group implemented conventional management mode and the study group implemented community hypertension chronic disease management mode.The management effect was compared between the two groups.Results:After management,the systolic blood pressure and diastolic blood pressure in the two groups were lower than those before management,and these indexes in the study group were lower than those in the control group,while the blood pressure compliance rate in the two groups was higher than that before management,and the rate in the study group was higher than that in the control group,and the difference was statistically significant(P<0.05).The daytime and nighttime systolic blood pressure fluctuation and diastolic blood pressure fluctuation values in the study group were lower than those in the control group,and the difference was statistically significant(P<0.001).After management,the scores of blood pressure monitoring management,medication management,diet management,and exercise management in the two groups were higher than those before management,and the scores in the study group were higher than those in the control group,and the difference was statistically significant(P<0.05).The satisfaction scores of the management practicability,operability,convenience and continuous improvement the study group were higher than those in the control group,and the difference was statistically significant(P<0.001).Conclusion:Community hypertension chronic disease management mode has significant application effect in elderly patients with hypertension,and can ameliorate blood pressure,red
