摘要
目的:通过Meta分析评估壳聚糖止血垫对经桡动脉行冠状动脉介入术(TRI)后的止血效果。方法:检索中国知网、万方数据库、维普数据库、PubMed、the Cochrane Library、Web of Science等中关于TRI术后病人应用壳聚糖止血垫的文献。由2名研究者严格按照纳入、排除标准独立筛选文献、质量评价和数据提取,采用RevMan 5.4软件进行Meta分析。结果:最终纳入9篇文献,涉及5 443例病人,其中试验组2 727例,对照组2 716例。Meta分析结果表明,与常规护理相比,壳聚糖止血垫能有效缩短TRI术后病人止血时间[SMD=-2.63,95%CI(-3.94,-1.32),P<0.000 1],降低桡动脉闭塞发生率[OR=0.63,95%CI(0.46,0.85),P=0.003]、前臂血肿发生率[OR=0.37,95%CI(0.27,0.50),P<0.000 01],差异均有统计学意义。结论:现有证据表明,与常规干预措施相比,壳聚糖止血垫可有效缩短经桡动脉行TRI术后病人的止血时间,降低桡动脉闭塞、前臂血肿等并发症的发生。
Objective:To evaluate the hemostatic effect of chitosan hemostatic pads after coronary intervention(TRI)via radial artery through Meta-analysis.Methods:The literatures on the application of chitosan hemostatic pad in patients after TRI were retrieved from CNKI,WanFang Database,VIP,PubMed,the Cochrane Library,Web of Science,etc.Literature selection,quality evaluation and data extraction were conducted independently by two researchers in strict accordance with inclusion and exclusion criteria.RevMan 5.4 software was used for Meta-analysis.Results:A total of 9 articles were included,involving 5443 patients,including 2727 patients in the experimental group and 2716 patients in the control group.The results of Meta-analysis showed that compared with conventional care,chitosan hemostatic pad could effectively shorten the hemostatic time of postoperative TRI patients(SMD=-2.63,95%CI-3.94--1.32,P<0.0001),and reduce the incidence of radial artery occlusion(OR=0.63,95%CI 0.46-0.85,P=0.003)and forearm hematoma(OR=0.37,95%CI 0.27-0.50,P<0.00001),with statistical significance.Conclusion:The available evidence shows that compared with conventional treatment,chitosan hemostatic pad can effectively shorten the hemostatic time of patients after transradial artery TRI,reduce the incidence of radial artery occlusion,forearm hematoma and other complications.
