摘要
目的:观察益气活血汤联合沙库巴曲缬沙坦对扩张型心肌病(DCM)并心力衰竭病人临床症状及心室重构的影响。方法:选取2020年1月—2023年1月DCM并心力衰竭病人99例,按随机数字表法分为对照组49例(给予沙库巴曲缬沙坦治疗)和观察组50例(益气活血汤联合沙库巴曲缬沙坦治疗),比较两组临床疗效,治疗前后中医症状积分、心排量指标[心输出量(CO)、心指数(CI)、左室射血时间(LVET)、左心作功指数(LCWi)、外周阻力(SVR)]、心室重构指标[左室射血分数(LVEF)、左室收缩末期内径(LVESD)、左室舒张末期内径(LVEDD)]、血清B型利钠肽原(NT-proBNP)、超敏C反应蛋白(hs-CRP)、心肌肌钙蛋白I(cTnI)水平。结果:观察组总有效率为88.00%,高于对照组的71.43%,差异有统计学意义(P<0.05)。治疗后,两组各项中医症状积分、SVR、LVESD、LVEDD、NT-proBNP、hs-CRP均低于治疗前(P<0.05),CO、CI、LVET、LCWi、LVEF均高于治疗前,且观察组优于对照组,差异均有统计学意义(P<0.05)。结论:益气活血汤联合沙库巴曲缬沙坦可改善DCM并心力衰竭病人临床症状、心功能及心室重构,安全性较高。
Objective:To observe the effects of boost Qi and invigorate blood decoction combined with sakubactril valsartan on clinical symptoms and ventricular remodeling in heart failure with dilated cardiomyopathy(DCM).Methods:Ninety-nine patients of heart failure with DCM from January 2020 to January 2023 were randomly divided into the control group(49 cases,treating with sakubactril valsartan)and observation group(50 cases,treating with boost Qi and invigorate blood decoction on the basis of sakubactril valsartan).The clinical efficacy of the two groups,Chinese medicine symptom score,cardiac displacement index[cardiac output(CO),cardiac index(CI),left ventricular ejection time(LVET),left ventricular work index(LCWi),peripheral resistance(SVR)],ventricular remodeling index[left ventricular ejection fraction(LVEF),left ventricular end-systolic diameter(LVESD),left ventricular end-diastolic diameter(LVEDD)],serum type B natriuretic peptide(NT-proBNP),hypersensitive C-reactive protein(hs-CRP),cardiac troponin I(cTnI)were compared between two groups before and after treatment.Results:The total effective rate of the observation group was significantly higher than that of the control group,and the difference was statistically significant(88.00%and 71.43%,P<0.05).After treatment,the scores of TCM symptoms,SVR,LVESD,LVEDD,NT-proBNP and hs-CRP in both groups decreased than those before treatment(P<0.05),CO,CI,LVET,LCWi and LVEF increased more than those before treatment,and the differences were statistically significant(P<0.05).Conclusion:Boost Qi and invigorate blood decoction combined with sakubactril valsartan for treating heart failure with DCM could improve clinical symptoms,cardiac function and ventricular remodeling,with high safety.
