摘要
目的:通过对比分析孕晚期胎膜早破产妇及正常产妇阴道微生态变化,探讨妊娠晚期孕产妇阴道微生态变化与妊娠胎膜早破结局之间的相关性,为预防和减少胎膜早破提供基础研究资料。方法:随机收集2022年4月至2023年12月符合纳入标准孕妇60例,分为胎膜早破组(实验组)和正常孕妇组(对照组)(n=30)。应用阴道炎联合检测试剂盒(干化学酶法)进行阴道分泌物微生态检测,对阴道微生态进行全面评价,对比两组之间的差异性。结果:实验组的阴道菌群密度及多样性Ⅰ、Ⅳ级居多,且阴道菌群的优势菌异常比例及病原体阳性几率、pH≥4.5、过氧化氢阳性、β-葡萄糖醛酸酶阳性、乙酰氨基葡萄糖苷酶阳性比例均高于对照组,差异有统计学意义(P<0.05),实验组的唾液酸苷酶阳性发生率比对照组的高,但差异无统计学意义(P>0.05)。结论:孕期阴道微生态失衡,胎膜早破的机会增加,从而使不良结局在母婴中发生的几率增大。
Objective:To explore the correlation between vaginal microecological changes in late pregnancy wom⁃en with premature rupture of membranes and normal women through comparative analysis of vaginal microecological changes in late pregnancy women and the outcome of premature rupture of membranes,in order to prevent and reduce fe⁃tal membranes.Premature rupture provides basic research information.Methods:60 pregnant women who met the inclu⁃sion criteria from April 2022 to December 2023 were randomly collected and divided into premature rupture of mem⁃branes group(experimental group)and normal pregnant women group(control group)(n=30).Use the vaginitis com⁃bined detection kit(dry chemical enzymatic method)to detect the microecology of vaginal secretions,conduct a compre⁃hensive evaluation of the vaginal microecology,and compare the differences between the two groups.Results:The test group had mostly vaginal flora density and diversity levelsⅠandⅣ,and the vaginal flora had an abnormal proportion of dominant bacteria and a positive probability of pathogens,pH≥4.5,positive hydrogen peroxide,positiveβ⁃glucuroni⁃dase,and acetaminophen.The positive proportion of glucosidase was higher than that of the control group,and the differ⁃ence was statistically significant(P<0.05).The positive incidence rate of sialidase in the test group was higher than that of the control group,but the difference was not statistically significant(P>0.05).Conclusion:The imbalance of vaginal microecology during pregnancy increases the chance of premature rupture of membranes,thereby increasing the chance of adverse outcomes in mother and baby.
