摘要
目的探讨膝骨性关节炎MRI软骨定量成像与西大略湖麦克马斯特大学骨关节炎指数(WOMAC)评分的相关性。方法选取2021年2月至2022年6月在抚州市第一人民医院接诊的20例膝骨性关节炎患者作为观察组,另选同期10例体检健康者作为对照组。所有受检者均接受MRI检查,对观察组患者进行WOMAC评分,同时对患者实施MRI软骨定量成像检测。根据全器官MRI评分将观察组分为观察1组(轻度,n=6)、观察2组(中度,n=6)、观察3组(重度,n=8)。采用Pearson相关分析膝关节骨性关节炎软骨全器官MRI评分、膝关节骨性关节炎软骨T2值与WOAMC评分的相关性,并比较各组膝关节软骨体积。结果Pearson相关分析结果显示,骨全器官MRI评分与临床表现WOMAC总评分(r=0.811,P<0.05)、关节疼痛评分(r=0.712,P<0.05)、关节僵硬评分(r=0.502,P<0.05)、关节活动度评分(r=0.824,P<0.05)呈正相关。软骨T2值与WOMAC总评分(r=0.499,P<0.05)、关节疼痛评分(r=0.376,P<0.05)、关节僵硬评分(r=0.362,P<0.05)、关节活动度评分(r=0.498,P<0.05)呈正相关。观察2组、观察3组髌骨、股骨、胫骨内侧髁、胫骨外侧髁体积均小于对照组及观察1组,观察3组髌骨、股骨体积均小于观察2组,观察1组髌骨、胫骨内侧髁体积均小于对照组,差异有统计学意义(P<0.05)。结论通过MRI半定量评价指标全器官MRI评分及定量评价指标T2值可以有效的评估膝骨性关节炎患者的临床表现,膝骨性关节炎MRI软骨定量成像与WOMAC评分密切相关。
Objective To explore the correlation between MRI cartilage quantitative imaging and Western Ontario and McMaster universities osteoarthritis index(WOMAC)score in knee osteoarthritis.Methods 20 patients with knee osteoarthritis treated in Fuzhou First People's Hospital from February 2021 to June 2022 were selected as the observation group,and 10 healthy people in the same period were selected as the control group.All subjects were examined with MRI,and the patients in the observation group were scored with WOMAC.At the same time,the patients were examined with MRI cartilage quantitative imaging.According to the whole organ MRI score,the pathology of the observation group was divided into observation group 1(mild,n=6),observation group 2(moderate,n=6)and observation group 3(severe,n=8).Pearson correlation analysis was used to investigate the correlation between whole organ MRI score,T2 value of knee osteoarthritis cartilage and WOAMC score,and compared the knee cartilage volume of each group.Results Pearson correlation analysis showed that MRI score of whole bone organs was positively correlated with WOMAC total score(r=0.811,P<0.05),joint pain score(r=0.712,P<0.05),joint stiffness score(r=0.502,P<0.05)and joint motion score(r=0.824,P<0.05);cartilage T2 value was positively correlated with WOMAC total score(r=0.499,P<0.05),joint pain score(r=0.376,P<0.05),joint stiffness score(r=0.362,P<0.05)and joint motion score(r=0.498,P<0.05).The volume of patella,femur,medial condyle of tibia and lateral condyle of tibia in the observation group 2 and observation group 3 were all lower than that in the control group and observation group 1,the volume of patella and femur in the observation group 3 was lower than that in the observation group 2,the volume of patella and medial condyle of tibia in the observation group 1 was lower than that in the control group,and the differences were statistically significant(P<0.05).Conclusion The clinical manifestations of patients with knee osteoarthritis can be effectively evaluated by MRI
