改良Kessler缝合法在指屈肌腱断裂修复中的临床应用及对患者手指屈伸运动和血运重建的影响

摘要 目的分析改良Kessler缝合法在指屈肌腱断裂修复中的应用效果及对患者手指屈伸运动和血运重建的影响。方法选取2021年4月至2022年4月于上饶市立医院就诊的76例指屈肌腱断裂患者作为研究对象,按照随机数字表法分为常规组与观察组,每组38例。常规组实施常规治疗方案,观察组实施改良Kessler缝合法治疗。比较两组临床疗效、手指屈伸功能、手指末梢血运恢复情况、并发症发生情况。结果观察组治疗总有效率高于常规组,差异有统计学意义(P<0.05);观察组患侧拇指、示指、中指、环指、小指总主动活动度系统(TAM)均高于常规组,差异有统计学意义(P<0.05);观察组健、患侧手指温差小于常规组,血管回流时间短于常规组,手指肿胀程度低于常规组,差异有统计学意义P<0.05);观察组并发症发生率低于常规组,差异有统计学意义(P<0.05)。结论改良Kessler缝合法治疗指屈肌腱断裂患者疗效显著,可促进手指屈伸功能恢复,加快血运重建,降低术后并发症发生风险,值得临床推广应用。 Objective To analyze the application effect of modified Kessler suture in the flexor tendon rupture of the fingers and the effect on flexion and extension movement of fingers and revascularization.Methods A total of 76 patients with flexor tendon rupture of the fingers who were treated in Shangrao Municipal Hospital from April 2021 to April 2022 were selected as the study subjects,and they were divided into the conven-tional group and the observation group according to the random number table method,with 38 cases in each group.The conventional group was treated with conventional treatment,and the observation group was treated with modified Kessler suture.The clinical effect,flexion and extension function of the fingers,recovery of blood flow of the finger endings and complications were compared between the two groups.Results The total effective rate in the observation group was higher than that in the conventional group,and the difference was statistically significant(P<0.05).To-tal active motion(TAM)of thumb,indicator finger,middle finger,ring finger and little finger on the affected side in the observation group were higher than those in the conventional group,the differences were statistically significant(P<0.05).The finger temperature difference between healthy and affected side in the observation group was lower than that in the conventional group,the vascular reflux time was shorter than that in the conventional group,and the finger swelling degree was lower than that in the conventional group,the differences were statistically significant(P<0.05).The incidence of complications in the observation group was lower than that in the conventional group,and the difference was statistically sig-nificant(P<0.05).Conclusion The modified Kessler suture is effective in the treatment of patients with flexor tendon rupture of the fingers,which can promote the recovery of flexion and extension function of the finger,accelerate the revascularization and reduce the risk of postoperative com-plications,and is worthy of clin
