摘要
目的观察采用血清相关指标结合肺炎严重程度评分(CURB-65评分表)用于评估急诊重症肺炎患者预后情况的效果.方法采用回顾性研究方法,收集2022年9月至2023年9月安溪县医院急诊科收治的重症肺炎患者共80例为研究对象.根据患者入院接受28d的治疗预后情况实施分组,生存组为55例,死亡组为25例.对比各组血清白细胞介素-6(Interleukin 6,IL-6)、C反应蛋白(C-reactionprotein,CRP)等指标情况.结果生存组CRP水平为(123.15±36.05)mg/L,死亡组为(117.15±35.56)mg/L,差异无统计意义(P>0.05);生存组白细胞介素-8(Interleukin-8,IL-8)水平为(43.05±7.76)pg/ml,死亡组为(56.27±10.63)pg/ml,且生存组血管生成素-2(Angiopoietin-2,Ang-2)表达水平低于死亡组(P<0.05);生存组IL-6水平为(32.61±6.89)pg/ml,死亡组为(41.49±10.48)pg/ml,且生存组肺炎严重程度评分明显低于死亡组,差异均有统计学意义(P<0.05);多因素Logistic回归分析结果显示,血清IL-6、IL-8、Ang-2、CURB-65评分为影响急诊重症肺炎患者出现不良预后的危险因素.结论血清IL-6、IL-8、Ang-2结合肺炎严重程度评分作为影响急诊重症肺炎患者预后情况的重要因素,对于辅助评估急诊重症肺炎患者预后情况具有重要的临床意义.
Objective To evaluate the prognostic value of serum markers combined with CURB-65 in pa-tients with acute severe pneumonia.Methods A retrospective study method was used to study eighty patients with severe pneumonia admitted to the emergency department of our hospital from September 2022 to September 2023.The patients were divided into the survival group(n=55)and the death group(n=25).The serum levels of In-terleukin-6(IL-6)and c-reaction protein(CRP)were IL6.Results The level of CRP in the survival group was(123.15±36.05)mg/l,and that in the death group was(117.15±35.56)mg/l,there was no significant difference between the two groups(P>0.05).The level of IL-8 in the survival group was(43.05±7.76)pg/ml,while that in the death group was(56.27±10.63)pg/ml.The expression level of Angiopoietin-2(Ang-2)in the survival group was lower than that in the death group(P<0.05).The level of IL-6 in the survival group was(32.61±6.89)pg/ml,and that in the death group was(41.49±10.48)pg/ml.The severity of pneumonia in the survival group was significantly lower than that in the death group,with statistically significant differences(P<0.05).Multivariable logistic regression showed that serum levels of IL-6,IL-8,Ang-2,and CURB-65 were risk factors for poor prognosis in patients with acute severe pneumonia.Conclusion The serum levels of IL-6,IL-8,Ang-2 and the severity of pneumonia may be important prognostic factors in patients with severe pneumonia,it is of great clinical significance to evaluate the prognosis of patients with severe pneu-monia in the emergency department.
