摘要
目的采用Meta分析比较锁定钢板与解剖钢板内固定治疗Pilon骨折的临床疗效和术后并发症发生率。方法通过万方、中国知网、维普、PubMed、Web of science中英文数据库检索有关锁定钢板与解剖钢板内固定治疗Pilon骨折的文献,2位研究员独立筛选符合条件的相关文献并进行质量分析,结局指标主要包括手术时间、骨折愈合时间、Tornetta评分、Burwell-Chamley放射学评分、并发症情况(皮肤坏死、感染、内固定物断裂、骨折延迟愈合、骨折不愈合)。结果纳入4篇文献,共258例,锁定钢板组132例,解剖钢板组126例。文献质量NOS量表评分:1篇8分,2篇7分,1篇6分。锁定钢板组手术时间较解剖钢板组短,骨折愈合时间较解剖钢板组短,术后总并发症发生率、骨折不愈合发生率低于解剖钢板组,差异有统计学意义(P<0.05)。两组Tornetta评分、Burwell-Chamley放射学评分、其他并发症(皮肤坏死、感染、内固定物断裂、骨折延迟愈合)发生率差异无统计学意义(P>0.05)。结论与解剖钢板内固定相比,锁定钢板内固定治疗Pilon骨折手术时间更短,骨折愈合更快,骨折不愈合发生率更低。
Objective To compare the clinical efficacy and postoperative complication rate of locking plate and anatomic plate in the treatment of Pilon fracture with a meta-analysis.Methods Literature on locking plate and anatomic plate internal fixation in the treatment of Pilon fracture was searched through Wanfang,CNKI,VIP,PubMed and Web of science databases in both Chinese and English.Two researchers independently screened eligible literature and conducted quality analysis.The outcome measures mainly included operative time,fracture healing time,Tornetta score,Burwell-Chamley radiological score,and complications(skin necrosis,infection,internal fixation rupture,delayed fracture union,and fracture nonunion).Results A total of 258 cases were included in 4 literatures,including 132 cases in the locking plate group and 126 cases in the anatomical plate group.NOS scale scores of literature quality:1 article scored 8,2 articles scored 7,and 1 article scored 6.The operation time and fracture healing time of the locking plate group were shorter than those of the anatomic plate group,and the incidence of postoperative complications and fracture nonunion was lower than that of the anatomic plate group,with statistical significance(P<0.05).There was no significant difference in Tornetta score,Burwell-Chamley radiology score and incidence of other complications(skin necrosis,infection,internal fixation rupture,and delayed fracture healing)between the two groups(P>0.05).Conclusion Compared with anatomic plate internal fixation,locking plate internal fixation for Pilon fractures has shorter operative time,faster fracture healing and lower incidence of fracture nonunion.
