人工智能在靶向视网膜光凝治疗糖尿病视网膜病变中的应用

摘要 目的:探讨人工智能(AI)在靶向视网膜光凝(TRP)治疗糖尿病视网膜病变(DR)中的应用。方法:将新余钢铁集团有限公司中心医院2020年4月—2023年3月收治的60例DR患者作为研究对象,按照随机数字表法分为两组,每组30例,两组均接受康柏西普治疗,对照组联合全视网膜光凝(PRP)治疗,研究组联合AI TRP治疗。观察两组临床疗效、视力改善状况、出血/渗血情况及并发症发生率。结果:研究组总有效率高于对照组,差异有统计学意义(P<0.05);术后1个月,两组黄斑区视网膜厚度、视神经纤维层厚度均较术前变薄(P<0.05),但两组对比差异均无统计学意义(P>0.05);术后1个月,研究组眼底出血/渗血情况优于对照组,差异有统计学意义(P<0.05);研究组角膜水肿、眼部疼痛的发生率均低于对照组(P<0.05),两组其余并发症发生率对比差异均无统计学意义(P>0.05)。结论:相较于PRP治疗DR,AI下TRP效果更好,出血/渗血情况更优,且不会影响患者术后视力的改善,安全性更高。 Objective:To investigate the application of artificial intelligence(AI)in the treatment of diabetic retinopathy(DR)with targeted retinal photocoagulation(TRP).Method:Sixty patients with DR admitted to Xin Steel Center Hospital from April 2020 to March 2023 were divided into two groups according to random number table method,with 30 patients in each group.Both groups were treated with Conbercept,the control group combined with panretinal photocoagulation(PRP)and the study group combined with AI TRP.The clinical effect,visual improvement,bleeding/oozing blood and complication rate of the two groups were observed.Result:The total effective rate of the study group was higher than that of the control group,the difference was statistically significant(P<0.05);1 month after operation,the thickness of retina and optic nerve fiber layer in both groups were thinner than those before operation(P<0.05),but there were no statistical significances between the two groups(P>0.05);1 month after surgery,the fundus bleeding/oozing blood in the study group was better than that in the control group,the difference was statistically significant(P<0.05);the incidences of corneal edema and eye pain in the study group were lower than those in the control group(P<0.05),there were no significant differences in the incidences of other complications between the two groups(P>0.05).Conclusion:Compared with PRP in the treatment of DR,TRP under AI has better effect,better bleeding/bleeding situation,and will not affect the improvement of postoperative visual acuity of patients,and has higher safety.
