摘要
目的在人工髋关节置换术手术室中实施术中保温护理方案,分析该护理方案的应用效果以及对预后的影响。方法选取本院2020年10月至2023年10月治疗的人工髋关节置换术患者80例,按照护理方式的个体差异性,将其分为对照组(常规手术室护理)、观察组(手术室护理+术中保温护理),每组40例。分析两组不同护理方式的临床应用效果。结果观察组干预后气管拔管时间、麻醉复苏室(PACU)滞留时间、完全清醒时间、麻醉恢复时间、肾上腺素(AD)、去甲肾上腺素(NE)、皮质醇(Cor)、凝血功能相关指标[凝血酶原时间(PT)、凝血酶时间(TT)、活化部分凝血活酶时间(ATPP)、纤维蛋白原(FIB)]、术后并发症(低体温、寒战、心律异常、低氧血症)发生率均低于对照组(P<0.05)。观察组术中(30 min、60 min)、手术结束时、术后(60 min)核心体温均高于对照组(P<0.05)。结论将术中保温护理运用于人工髋关节置换术手术室护理中,提升患者术后复苏效果,改善凝血功能,减轻应激反应,减少围手术期体温波动幅度,降低低体温、寒战等并发症发生率,有利于改善预后,值得推荐。
Objective Implement intraoperative thermal insulation nursing plan in the operating room of artificial hip replacement surgery,explore and analyze the application effect of this nursing plan and its impact on prognosis.Methods Eighty patients who underwent artificial hip replacement surgery in our hospital from October 2020 to October 2023 were selected.According to individual differences in nursing methods,they were divided into a control group(conventional operating room nursing)and an observation group(operating room nursing+intraoperative thermal insulation nursing),with 40 cases per group.Compare and analyze the clinical application effects of different nursing methods.Results After intervention,the observation group observed tracheal extubation time,anesthesia recovery room(PACU)retention time,complete wakefulness time,anesthesia recovery time,adrenaline(AD),norepinephrine(NE),cortisol(Cor),coagulation function related indicators[prothrombin time(PT),thrombin time(TT),activated partial thromboplastin time(ATPP),fibrinogen(FIB)],the incidence of postoperative complications(hypothermia,chills,arrhythmia,hypoxemia)was lower than in the control group(P<0.05).The core body temperature of the observation group was higher than that of the control group during surgery(30 minutes,60 minutes),at the end of surgery,and after surgery(60 minutes)(P<0.05).Conclusions Applying intraoperative thermal insulation care in the operating room of artificial hip replacement surgery can improve the postoperative recovery effect of patients,improve coagulation function,reduce stress reactions,reduce perioperative temperature fluctuations,and lower the incidence of complications such as hypothermia and chills.It is beneficial for improving prognosis and is worth recommending.
