摘要
目的:探讨泄热降逆汤配合兰索拉唑肠溶胶囊用于反流性食管炎肝胃郁热证的临床效果。方法:选取2018年5月-2020年8月我院收治的90例反流性食管炎肝胃郁热证患者作为研究对象,以抽签法分为两组,对照组(n=45)采用兰索拉唑肠溶胶囊治疗,观察组(n=45)另给予泄热降逆汤治疗,对比两组治疗效果。结果:观察组治疗总有效率(95.56%,43/45)明显高于对照组(80.00%,36/45)(P<0.05);治疗后观察组中医主证、次证评分及食管黏膜形态评分明显低于对照组(P<0.05);治疗后观察组食管黏膜氧化应激指标[丙二醇(PEG)、一氧化氮(NO)]表达低于对照组,总超氧化物歧化酶(T-SOD)表达高于对照组(P<0.05);治疗后观察组胃泌素(Gas)、胃动素(MOT)及血管活性肠肽(VIP)表达高于对照组(P<0.05);治疗后观察组肿瘤坏死因子-α(TNF-α)、干扰素-γ(IFN-γ)、白细胞介素-6(IL-6)低于对照组,白细胞介素-4(IL-4)高于对照组(P<0.05);治疗结束后随访6个月,观察组复发率(2.22%,1/45)明显低于对照组(13.33%,6/45),差异显著(P<0.05)。结论:泄热降逆汤配合兰索拉唑肠溶胶囊用于反流性食管炎肝胃郁热证的治疗效果显著,能够调节免疫,改善临床症状及食管黏膜损伤,促肠胃蠕动,值得临床推广。
Objective:To explore the clinical effect of Xiere jiangni decoction combined with lansoprazole enteric-coated capsule in the treatment of reflux esophagitis with liver-stomach heat stagnation syndrome.Methods:90 patients with liver-stomach stagnation-heat syndrome of reflux esophagitis treated in our hospital from May 2018 to August 2020 were divided into two groups by drawing lots.The control group(n=45)was treated with lansoprazole enteric-coated capsule,and the observation group(n=45)was treated with Xiere Jiangni decoction.The therapeutic effects of the two groups were compared.Results:The total effective rate of treatment in the observation group(95.56%,43/45)was significantly higher than that in the control group(80.00%,36/45)(P<0.05).After treatment,the scores of main syndrome and secondary syndrome of TCM and the morphology of esophageal mucosa in the observation group were significantly lower than those in the control group(P<0.05).After treatment,the expressions of propanediol(PEG),nitric oxide(NO),total superoxide dismutase(T-SOD),gastrin(Gas),motilin(MOT)and vasoactive intestinal peptide(VIP)in the observation group were lower than those in the control group(P<0.05).After treatment,the levels of tumor necrosis factor-α(TNF-α),interferon-action-γ(IFN-γ)and interleukin-6(IL-6)in the observation group were lower than those in the control group,while the level of interleukin-4(IL-4)in the observation group was higher than that in the control group(P<0.05).After 6 months of follow-up after treatment,the recurrence rate in the observation group(2.22%,1/45)was significantly lower than that in the control group(13.33%,6/45).Conclusion:Xiere jiangni decoction combined with lansoprazole enteric-coated capsule is effective in the treatment of reflux esophagitis with liver-stomach heat stagnation syndrome,which can regulate immunity,improve clinical symptoms and esophageal mucosal injury,and promote gastrointestinal peristalsis,which is worth popularizing.
