摘要
目的 研究改良仰卧位牵引架复位防旋股骨近端髓内钉(Proximal Femoral Nail Antirotation,PFNA)内固定治疗老年股骨转子间骨折的效果。方法 选取2020年1月—2023年3月盱眙县人民医院收治的109例股骨转子间骨折老年患者为研究对象,按随机数表法分为对照组(n=54)和研究组(n=55)。对照组行常规仰卧位牵引床辅助下复位PFNA内固定治疗,研究组行改良仰卧位牵引架复位PFNA内固定治疗。对比两组患者各项临床指标、髋关节功能、并发症发生情况。结果 研究组手术时间[(50.26±7.91)min]短于对照组[(63.81±10.21)min],术中出血量[(75.91±12.57)mL]少于对照组[(125.64±15.27)mL],差异有统计学意义(t=7.754、18.578,P均<0.001)。研究组住院时间[(10.52±2.61)d]短于对照组[(12.03±2.43)d],差异有统计学意义(P<0.05)。研究组髋关节功能优良率(85.45%)高于对照组(66.66%),差异有统计学意义(P<0.05)。两组并发症发生情况对比,差异无统计学意义(P>0.05)。结论 改良仰卧位牵引架复位PFNA内固定治疗老年股骨转子间骨折效果显著,并能够改善患者髋关节功能,同时安全性较高,有较高临床价值。
Objective To study the effect of modified supine traction frame repositioning proximal femoral nail antirotation(PFNA) internal fixation in the treatment of intertrochanteric fracture of the femur in the elderly.Methods 109 elderly patients with intertrochanteric fracture of the femur admitted to Xuyi County People's Hospital from January 2020 to March 2023 were selected as the study objects,according to the random number table method,and they were divided into the control group(n=54) and the study group(n=55).The control group was treated with PFNA internal fixation with the assistance of conventional supine traction bed,and the study group was treated with PFNA internal fixation with modified supine traction frame.The clinical indicators,hip function and complications of the two groups were compared.Results The operation time in the study group [(50.26±7.91) min] was shorter than that in the control group [(63.81±10.21) min] and intraoperative bleeding in the study group [(75.91±12.57) mL] was less than that in the control group [(125.64±15.27) mL],and the differences were statistically significant(t=7.754,18.578,both P<0.001).The hospitalization time of the study group [(10.52±2.61) d] was shorter than that of the control group [(12.03±2.43) d],the difference was statistically significant(P<0.05).The rate of excellent hip function of the study group(85.45%) was higher than that of the control group(66.66%),the difference was statistically significant(P<0.05).There was no statistically significant difference of the complication rates between the two groups(P>0.05).Conclusion The effect of modified supine traction frame PFNA internal fixation in treating intertrochanteric fracture of the femur in the elderly is remarkable,and it can improve the hip function of the patients,meanwhile,it is safe and can has clinical value.
