MR颅脑磁敏感加权成像评估高血压分级与脑内微出血的相关性 被引量:1

Study on the correlation between hypertensive grading and cerebral microbleeds evaluated by susceptibility weighted imaging

摘要 目的探讨MR颅脑磁敏感加权成像(SWI)评估高血压分级与脑内微出血的相关性。方法选取在我院接受临床诊断和治疗的高血压患者300例,根据血压水平分为1级高血压组、2级高血压组、3级高血压组。所有患者均行MR颅脑SWI检查,比较三组患者脑微出血(CMBs)的检出数量,并分析CMBs检出数量与高血压分级的相关性。采用MedCalc软件计算SWI诊断不同高血压分级患者CMBs的敏感度、特异度、阳性预测值、阴性预测值。根据CMBs检查结果对CMBs进行分型,分析高血压分级和CMBs分型的相关性。结果3级高血压组患者CMBs阳性率和CMBs检出数量均高于1级高血压组和2级高血压组,差异有统计学意义(P<0.05)。高血压分级和CMBs检出数量呈正相关性(r>0,P<0.05)。SWI对不同高血压分级患者CMBs的诊断敏感度、特异度、阳性预测值、阴性预测值比较差异无统计学意义(P>0.05)。3级高血压组患者Ⅰ型CMBs、Ⅱ型CMBs、Ⅴ型CMBs的阳性率均高于1级高血压组和2级高血压组,差异有统计学意义(P<0.05)。Spearman相关检验显示,高血压分级和CMBs分型无明显相关性(P>0.05)。结论脑内微出血和高血压病情密切相关,MR颅脑SWI对评估不同高血压分级患者CMBs的分布情况具有重要的应用价值。 Objective To explore the correlation between hypertensive grading and cerebral microbleeds evaluated by sus‐ceptibility weighted imaging.Methods 300 patients with hypertension who received clinical diagnosis and treatment in our hos‐pital were selected and divided into grade 1 hypertension group,grade 2 hypertension group,and grade 3 hypertension group ac‐cording to blood pressure levels.All patients underwent MR brain SWI examination.The number of cerebral microbleeds(CMBs)detected in the three groups were compared,and the correlation between the number of CMBs detected and the classifica‐tion of hypertension was analyzed.The sensitivity,specificity,positive predictive value,and negative predictive value of SWI di‐agnosis of CMBs in patients with different grades of hypertension were calculated by MedCalc.According to the results of CMBs ex‐amination,CMBs were classified,and the correlation between hypertension classification and CMBs classification was analyzed.Results The positive rate of CMBs and the number of CMBs in grade 3 hypertension group were higher than those in grade 1 hy‐pertension group and grade 2 hypertension group(P<0.05).The grade of hypertension was positively correlated with the number of CMBs(r>0,P<0.05).There was no significant difference in sensitivity,specificity,positive predictive value and negative predic‐tive value of SWI in the diagnosis of CMBs in patients with different hypertension grades(P>0.05).The positive rates of type I CMBs,type II CMBs and type V CMBs in grade 3 hypertension group were higher than those in grade 1 hypertension group and grade 2 hypertension group(P<0.05).Spearman correlation test showed that there was no significant correlation between hyper‐tension classification and CMBs classification(P>0.05).Conclusion CMBs are closely related to the condition of hypertension.SWI has important application value in evaluating the distribution of CMBs in patients with different grades of hypertension.
