基于NHANES数据库老年人听力损失和认知功能的相关性研究

摘要 目的探讨60~69岁老年人听力损失和认知功能(整体及特定领域)之间的相关性。方法本研究选取美国国家健康和营养调查(National Health and Nutrition Examination Survey,NHANES)2011-2012年的数据,筛选出307名年龄在60~69岁、纯音测听和认知功能等数据完整的受试者。以性别、年龄、教育程度、家庭贫困收入比、吸烟、使用助听器、糖尿病史、自我评估的听力状况、高密度脂蛋白、纯音测听阈值得分等作为自变量,以认知功能综合得分作为因变量,进行多元线性回归分析。结果在307名受试者中,以较好耳500 Hz、1000 Hz、2000 Hz、4000 Hz四个频率气导听阈的平均值(以≤25 dB为准),分为听力正常组(n=188)和听力损失组(n=119)。听力正常组的认知功能综合得分显著高于听力损失组(P<0.001),且在认知功能特定领域中,CERAD和DSST得分也显著高于听力损失组(P<0.001),动物流利度测试得分有差异(P=0.035)。多元线性回归分析结果表明,纯音测听阈值得分与认知功能得分存在负相关(相关系数β=-0.279,95%CI为-0.450~-0.108),年龄、性别、教育程度、家庭贫困收入比、种族、冠心病等也是影响老年人认知功能的因素。结论60~69岁老年人的听力损失与其认知功能(整体及特定领域)存在相关性。 Objective This study aims to investigate the relationship between hearing loss and cognitive function(overall and specific domains)in older adults.Methods Data on pure tone audiometry and cognitive function from 307 participants aged 60-69 years in the National Health and Nutrition Examination Survey(NHANES)2011-2012 were selected.Gender,age,education level,family poverty income ratio,smoking,use of hearing aids,history of diabetes,self-assessed hearing status,high-density lipoprotein,pure tone audiometry threshold score and other factors were used as independent variables,and cognitive function composite score was used as dependent variable.Multiple linear regression analysis was performed.Results Based on the average value of air conduction thresholds at four frequencies(500,1000,2000,4000 Hz)in the better ear(≤25 dB),188 participants were classified as normal-hearing group and 119 as hearing-impaired group.The normal-hearing group had a significantly higher cognitive function composite score than the hearing-impaired group(P<0.001),as well as significantly higher scores in specific domains of cognitive function such as CERAD and DSST(P<0.001).There was also a difference in animal fluency test score(P=0.035).The results of multiple linear regression analysis showed that pure tone audiometry threshold score was negatively correlated with cognitive function score(correlation coefficientβ=-0.279,95%CI=-0.450 to-0.108),and age,gender,education level,family poverty income ratio,high-density lipoprotein,triglyceride were all influencing factors of cognitive function status in older adults.Conclusion Hearing loss was related to cognitive fuction(overall and specific domains)in order adults.
