腺苷在STEMI患者急诊PCI术中的研究进展

摘要 ST段抬高型心肌梗死(ST-elevation myocardial infarction,STEMI)是导致心源性死亡的主要原因,而经皮冠状动脉介入治疗(PCI)是治疗STEMI重要的再灌注治疗方法之一,但其所致的再灌注损伤(ischemia reperfusion injury,IRI)伤会增加心肌梗死面积,进而使心肌梗死后心力衰竭发生率和死亡率升高。腺苷在多个动物实验及临床实验中被证明,作为一种内源性物质,与其受体能减轻缺血后IRI,在体内多个环节中起到心肌保护作用,并在临床上表现为无复流、主要心血管不良事件(MACE)的减少及微循环的改善等。但在最佳的临床适应证、使用方式、剂量、持续时间以及潜在获益的确切机制等方面,腺苷仍有较多争议。
