摘要
目的 探讨AirSeal智能气腹系统在机器人辅助腹腔镜根治性前列腺切除手术中的应用效果。方法 选择2019年12月至2022年12月机器人辅助腹腔镜根治性前列腺切除手术患者100例,随机分成观察组与对照组各50例,观察组术中采用AirSeal智能气腹系统维持气腹,对照组术中采用STORZ或OLYMPUS等标准系统自动气腹机维持气腹,比较两组手术持续时间、术中出血量、术后住院时间、住院总费用、术后4h、8h、24h肩部疼痛程度及医师满意度。结果 两组患者手术均顺利完成,无严重并发症,观察组手术持续时间(150.76±32.47)min,短于对照组(163.32±29.54)min,差异有统计学意义(P=0.046);术后8 h观察组肩部疼痛程度比对照组轻,差异有统计学意义(P=0.041);观察组各维度满意度均高于对照组,差异有统计学意义(P<0.05)。结论 AirSeal智能气腹系统在机器人辅助腹腔镜根治性前列腺切除手术中能缩短手术时间,缓解患者术后肩部疼痛,提高术者满意度,有一定的临床应用价值。
Objective To investigate the application effect of AirSeal intelligent pneumoperitoneum system in robot-assisted laparoscopic radical prostatectomy(RALP).Methods 100 patients with RALP since December 2019 to December 2022.Patients were randomly divided into the observation group and the control group with 50 patients in each group.The observation group was maintained pneumoperitoneum by AirSeal intelligent pneumoperitoneum system.The control group were performed with the automatic pneumoperitoneum machine of standard system such as STORZ or OLYMPUS to maintain pneumoperitoneum.The duration of operation,intraoperative blood loss,post-operative hospitalization,total hospitalization cost,postoperative shoulder pain degree of 4.0 h,8.0 h,24.0 h and doctor's satisfaction were compared and recorded between the two groups.Results Surgery was successfully completed in both groups without serious complications.The operation duration of the observation group(150.76±32.47)was shorter than the control group(163.32±29.54),with statistical significance(P=0.046).The degree of shoulder pain in the observation group was less than the control group at 8.0 h after surgery(P=0.041).The satisfaction of each dimension in the observation group was higher than that in the control group(P<0.05).Conclusion AirSeal intelligent pneumoperitoneum system in RALP can shorten the operation time,relieve postoperative shoulder pain,improve the surgeon's satisfaction,and has certain clinical application value.
