摘要
目的探讨与抗结核药物性肝损伤相关的危险因素,减少肝损伤的发生。方法2019年随机抽取呼伦贝尔市第二人民医院治疗的158例肺结核患者,采用现况研究方法分析性别、年龄、民族、BMI分级、平均睡眠、以肉或奶食为主、是否使用一线药物、户籍类型、婚姻状况、文化程度及职业等因素对抗结核药物性肝损伤发生的影响,将全部研究对象随机分为训练样本和测试样本,利用训练样本筛选出对抗结核药物性肝损伤危险因素,构建人工神经网络诊断模型,用ROC曲线下面积、灵敏度及特异度评价其诊断性能。结果年龄段及平均睡眠时间是肺结核患者肝损伤发生的独立危险因素,人工神经网络模型重要性预测显示,年龄是抗结核药物性肝损伤的主要危险因素。人工神经网络模型训练组预测正确率为63.1%,测试组预测正确率为53.2%,人工神经网络模型的ROC曲线下面积为0.681。结论年龄和平均睡眠时间是抗结核药物性肝损伤的高危因素。
Objective To explore the risk factors for liver injury caused by anti-tuberculosis drugs and thus reduce the incidence of liver injury.Methods A total of 158 patients with tuberculosis treated in Hulunbuir Second People's Hospital were randomly selected.According to the cross-sectional study,the effects of gender,age,ethnicity,body mass index(BMI)classification,average sleeping duration,meat or milk consumption,use of first-line drugs,household registration type,marital status,education level and occupation on the occurrence of anti-tuberculosis drug-induced liver injury were analyzed.Patients were randomly divided into the training dataset and testing dataset.Risk factors for liver injury caused by anti-tuberculosis drugs were screened using the training dataset.An artificial neural network model was then created,and its diagnostic performance was assessed by calculating the area under the receiver operating characteristic(ROC)curve(AUC),sensitivity and specificity.Results Age and average sleeping duration were independent risk factors for liver injury in pulmonary tuberculosis patients.An artificial neural network model used to predict the significance showed that age was the main risk factor for liver injury induced by anti-tuberculosis drugs.The accuracy rate of the artificial neural network model in the training dataset and testing dataset was 63.1%and 53.2%,respectively.The AUC of the artificial neural network model was 0.681.Conclusion Age and average sleeping duration are high risk factors for liver injury induced by antituberculosis drugs.
