摘要
目的探讨血清趋化素(Chemerin)在妊娠期糖尿病(GDM)孕妇中的变化,分析其与胰岛素抵抗的关系。方法选择2019年3月至2020年2月深圳市龙岗中心医院收治的50例GDM孕妇作为观察组,并选择同期接诊的血糖筛查或75 g葡萄糖耐量(OGTT)检查正常的50例孕妇作为对照组。比较两组孕妇的甘油三酯(TG)、总胆固醇(TC)、空腹血糖(FPG)、胰岛素抵抗指数(HOMA-IR)及血清Chemerin水平,并采用Pearson法分析血清Chemerin水平与HOMA-IR的相关性。结果观察组孕妇的TG、TC、FPG、HOMA-IR分别为(2.24±0.41)mmol/L、(5.30±0.68)mmol/L、(6.02±0.58)mmol/L、2.30±0.42,均明显高于对照组的(1.62±0.33)mmol/L、(4.76±0.57)mmol/L、(4.29±0.41)mmol/L、1.23±0.37,差异均有统计学意义(P<0.05);观察组孕妇的血清Chemerin水平为(8.78±1.89)mg/L,明显高于对照组的(4.54±1.21)mg/L,差异具有统计学意义(P<0.05);Pearson相关性分析结果显示,血清Chemerin水平与HOMA-IR呈正相关(r=0.619,P<0.05)。结论血清Chemerin在GDM孕妇中明显升高,且与胰岛素抵抗有明显相关性。
Objective To investigate the changes of serum chemerin in pregnant women with gestational diabetes mellitus(GDM),and analyze its relationship with insulin resistance.Methods Fifty pregnant women with GDM admitted to Shenzhen Longgang Central Hospital from March 2019 to February 2020 were selected as the observation group,and fifty pregnant women who were normal in blood glucose screening or 75 g oral glucose tolerance test(OGTT)at the same period were selected as the control group.The triglyceride(TG),total cholesterol(TC),fasting plasma glucose(FPG),Homeostasis Model Assessment of Insulin Resistance Index(HOMA-IR),and serum chemerin levels were compared between the two groups,and the correlation between serum Chemerin levels and HOMA-IR was analyzed by Pearson method.Results The TG,TC,FPG,and HOMA-IR in the observation group were(2.24±0.41)mmol/L,(5.30±0.68)mmol/L,(6.02±0.58)mmol/L,2.30±0.42,respectively,which were significantly higher than(1.62±0.33)mmol/L,(4.76±0.57)mmol/L,(4.29±0.41)mmol/L,1.23±0.37 in the control group(P<0.05).The serum chemerin level in the observation group was(8.78±1.89)mg/L,which was significantly higher than(4.54±1.21)mg/L in the control group(P<0.05).Pearson correlation analysis showed that serum chemerin level was positively correlated with HOMA-IR(r=0.619,P<0.05).Conclusion The serum chemerin is significantly elevated in GMD pregnant woman and significantly correlated with insulin resistance.
