基于功能磁共振成像技术探讨推拿干预腰椎间盘突出症的脑中枢镇痛机制

Exploration on the central cerebral analgesic mechanism of Tuina intervention in lumbar disc herniation based on fMRI

摘要 腰椎间盘突出症(LDH)是临床常见病,腰痛和功能障碍是其主症,可伴随下肢痛、麻或无力。脑功能磁共振成像(fMRI)有较高的时空分辨率和精确度,有安全、无创、无辐射等特点,在人体和动物的脑功能活体检测中应用逐渐增多,可揭示LDH疼痛脑中枢机制。推拿治疗LDH镇痛效应显著,但其脑中枢镇痛机制尚未引起足够重视。基于fMRI探讨LDH疼痛脑机制以及推拿干预LDH脑中枢镇痛机制,从低频振幅、功能连接、局部一致性、默认网络等指标进行多维度分析,可为临床诊疗提供参考。 Lumbar disc herniation(LDH)is a common clinical disease,low back pain and dysfunction are the main symptoms,which may be accompanied by lower limb pain,numbness,or weakness.Brain functional magnetic resonance imaging(fMRI)has high spatio-temporal resolution and accuracy,and has the characteristics of safety,non-invasiveness and non-radiation,which are gradually increasing in the application of in vivo brain function testing in humans and animals,and can revel the central brain mechanism of LDH pain.The analgesic effect of Tuina in treating LDH is significant,but its brain analgesic mechanism has not yet attracted sufficient attention.Based on the fMRI to explore the LDH pain brain mechanism and the central analgesic mechanism of Tuina intervention in LDH brain,multi-dimensional analysis from low-frequency amplitude,functional connectivity,regional homogeneity and default network and other indicators can provide reference for clinical diagnosis and treat.
