摘要
目的探讨中药熏蒸联合离子导入术前干预对改善白内障术后干眼的作用。方法将2020年1月—2020年6月北京中医药大学枣庄医院眼科收治的111例白内障患者分为治疗组及对照组,并将治疗组和对照组患者按证型再分为肝肾不足组和肝热上扰组。治疗组术前3 d予以中药外治,对照组术前不进行眼表的治疗。观察各组术前及术后1周、1个月、3个月的干眼症状积分、泪液分泌量、角膜荧光染色和泪膜破裂时间的情况。结果白内障患者术后1个月干眼发病率显著升高。治疗组比对照组术后各时段干眼发病率降低(P<0.05),且肝热上扰组低于肝肾不足组(P<0.05)。结论白内障手术会增加干眼的发生率,中医外治术前干预对白内障术后1个月干眼的发生率改善显著,且对肝热上扰证效果优于肝肾不足证。
Objective To explore the effect of traditional Chinese medicine fumigation combined with iontophoresis preoperative intervention on improving dry eye after cataract surgery.Methods From January 2020 to June 2020,111 cases of cataract patients admitted to the department of ophthalmology of Zaozhuang Hospital of Beijing University of Chinese Medicine were divided into treatment group and control group,and the treatment group and control group were further divided into liver and kidney deficiency group and liver heat disturbance group according to syndrome types.The treatment group was treated with traditional Chinese medicine 3 days before the operation,and the control group was not treated with the ocular surface before the operation.The dry eyes symptom score and tear secretion,corneal fluorescence staining and tear film rupture time of each group were observed before the operation,1 week,1 month,and 3 months after the operation.Results The incidence of dry eye increased significantly one month after cataract surgery.Compared with the control group,the incidence of dry eye was lower in the treatment group at various times after the operation(P<0.05),and the liver heat disturbance group was better than the liver and kidney deficiency group(P<0.05).Conclusion Cataract surgery will increase the incidence of dry eye.Preoperative intervention of traditional Chinese medicine has significantly improved the incidence of dry eye at 1 month after cataract surgery,and it is better for liver heat disturbance than for liver and kidney deficiency type.
