摘要
目的探究多层螺旋电子断层扫描(multi-slice spiral computed tomography,MSCT)在食管癌患者术前分期中的应用价值。方法回顾性分析南阳医学高等专科学校第一附属医院2021年3月至2023年3月收治的96例食管癌患者的临床资料和MSCT影像资料,以病理学检查结果为金标准,判断MSCT对食管癌术前分期诊断的准确率。结果病理学检查显示T1期18例,T2期26例,T3期29例,T4期23例;N0期52例,N1~N3期44例;MSCT检查诊断T1期14例,T2期23例,T3期27例,T4期22例,诊断的准确率分别为77.78%、88.46%、93.10%、95.65%,与金标准相比诊断T分期总准确率为89.58%,诊断一致性检验Kappa=0.779(P<0.05);诊断N0期40例,N1~N3期36例,诊断的准确率分别为76.92%、81.82%,与金标准相比诊断N分期总准确率为79.17%,诊断一致性检验Kappa=0.583(P<0.05)。结论MSCT对食管癌患者术前分期的诊断具有一定的价值,临床上可据此制定治疗方案和评估预后情况。
Objective To explore the application value of multi-slice spiral computed tomography(MSCT)in preoperative staging of esophageal cancer.Methods A retrospective analysis was performed on the clinical data and MSCT imaging data of 96 patients with esophageal cancer admitted to our hospital between March 2021 and March 2023.Using pathological examination results as the gold standard,the accuracy of MSCT in preoperative staging diagnosis of esophageal cancer was determined.Results Pathological examination showed that there were 18 cases at stage T1,26 cases at stage T2,29 cases at stage T3 and 23 cases at stage T4.There were 52 cases at stage N0 and 44 cases at stage N1-N3.MSCT showed that there were 14 cases at stage T1,23 cases at stage T2,27 cases at stage T3 and 22 cases at stage T4,with diagnostic accuracy of 77.78%,88.46%,93.10%,and 95.65%,respectively.Compared with the golden standard,total diagnostic accuracy rate of MSCT for T staging was 89.58%,and the diagnostic consistency test Kappa=0.779(P<0.05).MSCT showed that there were 40 cases at stage N0 and 36 cases at stage N1-N3,and diagnostic accuracy rates were 76.92%and 81.82%,respectively.Compared with the golden standard,total diagnostic accuracy rate of MSCT for N staging was 79.17%,and the diagnostic consistency test Kappa=0.583(P<0.05).Conclusion MSCT has certain diagnostic value for preoperative staging in patients with esophageal cancer,and could be applied to formulate treatment plans and evaluate prognosis in clinic.
