摘要
目的:对比髓内针内固定与钉板固定治疗第四或(和)五掌骨颈骨折患者的效果。方法:回顾性收集2008年12月~2021年12月本院97例第四或(和)五掌骨颈骨折患者病例资料,按手术方案不同分成两组。对照组接受钉板固定治疗,实验组接受髓内针内固定治疗,对比两组疗效。结果:实验组手术优良率100%较对照组的84%高(P<0.05);术后1个月、3个月实验组VAS评分较对照组低(P<0.05);实验组术中失血量较对照组少,手术耗时、骨折愈合时间较对照组短(P<0.05);实验组关节功能恢复优良率92%较对照组的82%高(P<0.05);术后1个月、3个月实验组血清OC含量较对照组高,血清β-CTX含量较对照组低(P<0.05)。结论:与钉板固定治疗第四或(和)五掌骨颈骨折患者相比,应用髓内针内固定治疗在优化手术指标,促进关节功能恢复,改善手功能,减轻疼痛感方面更具优势,且更有助于促进骨折愈合。
Objective:To compare the efficacy of intramedullary needle fixation and nail plate fixation in the treatment of fourth or(and)fifth metacarpal neck fracture.Methods:Retrospective collection of case data from 97 patients with fourth or(and)fifth metacarpal neck fractures in our hospital from December 2008 to December 2021,divided into two groups according to different surgical plans.The control group received nail plate fixation treatment,while the experimental group received intramedullary nail fixation treatment.The therapeutic effects of the two groups were compared.Results:The operative rate of the experimental group was 100%higher than that of the control group 84%(P<0.05).The VAS score of the experimental group was lower than that of the control group at 1 month and 3 months after operation(P<0.05).The blood loss in the experimental group was less than that in the control group,and the operation time and fracture healing time were shorter(P<0.05).The recovery rate of joint function in the experimental group was 92%higher than that in the control group(82%)(P<0.05).The serum OC content of the experimental group was higher than that of the control group at 1 month and 3 months after operation,and the serumβ-CTX content was lower than that of the control group(P<0.05).Conclusions:Compared with nail plate fixation for patients with fourth or(and)fifth metacarpal neck fractures,intramedullary pin fixation is more advantageous in optimizing surgical indicators,promoting joint function recovery,improving hand function,alleviating pain,and promoting fracture healing.
