拮抗剂方案中不同时期黄体生成素水平对体外受精-胚胎移植妊娠结局的影响研究

摘要 研究拮抗剂方案中卵巢正常反应的患者不同时期的黄体生成素(luteinizing hormone,LH)水平对体外受精-胚胎移植(IVF-ET)妊娠结局的影响。选取贵州省人民医院生殖医学科2022年3月—2023年3月收治的采用拮抗剂方案促排卵且卵巢功能正常的102例患者为研究对象。选择LH的四个时期:基础的LH水平、加西曲瑞克日的LH水平、促排卵药物(Gn)使用第8天后LH的水平、扳机日(HCG日)的LH水平。将基础LH、加西曲瑞克日LH、Gn第8天LH、HCG日LH四个测定时间点的LH数值按从低到高分为三组,A组(LH水平为前25%),B组(LH水平为中间的25%~75%),C组(LH水平为后25%)。比较各组间的一般资料、Gn使用天数、Gn使用量、获卵数、优胚率、妊娠率等指标。研究发现,在拮抗剂方案促排过程中,基础LH值及HCG日LH值偏低可能是COH过程中的Gn天数增多和Gn用量增多引起的,且基础LH的水平可能与早期卵泡的募集相关,促排卵过程中LH水平值的高低对最终的妊娠率没有影响。 Understand the impact of luteinizing hormone(LH)levels at different stages on pregnancy outcomes in in vitro fertilization embryo transfer(IVF-ET)in patients with normal ovarian response in antagonist regimens.102 patients admitted to the Reproductive Medicine Department of Guizhou Provincial People’s Hospital from March 2022 to March 2023 who were treated with an antagonist regimen to induce ovulation and had normal ovarian function were selected as the study subjects.Select four LH stages:baseline LH level,LH level on day of treatment with gacetrexed,LH level on day 8 of ovulation inducing drug(Gn)use,and LH level on trigger day(HCG day).Divide the LH values at four measurement time points,namely basic LH,Jiaxiqurike daily LH,Gn day 8 LH,and HCG day LH,into three groups from low to high:Group A(LH level in the top 25%),Group B(LH level in the middle 25%to 75%),and Group C(LH level in the bottom 25%).Compare general information,Gn usage days,Gn usage amount,number of retrieved eggs,excellent embryo rate,pregnancy rate,and other indicators between groups.Research has found that during the ovulation induction process with antagonist regimens,lower basal LH values and HCG daily LH values may be caused by an increase in Gn days and Gn dosage during COH,and basal LH levels may be related to early follicle recruitment.The level of LH during ovulation induction has no effect on thefinal pregnancy rate.
