摘要
目的探究多孔金属板辅助下微创治疗急性闭合性跟腱断裂的临床疗效。方法回顾性研究2016年3~12月就诊的急性闭合性跟腱断裂患者共19例,采用多孔金属板辅助下微创跟腱修补术,统计手术切口长度、手术时间、术中出血量、并发症、术中穿刺次数、疼痛视觉模拟评分(VAS)、美国足与踝关节协会踝与后足功能评分(AOFAS)、跟腱完全断裂评分(ATRS)和Kaikkonen评分。结果患者平均切口长度(2.61±0.23)cm,平均手术时间(41.42±4.93)min,平均术中出血量(13.22±2.07)mL术中平均穿刺7(6,8)次,均无手术并发症。患者术前、术后第1天、术后第7天VAS评分比较差异无统计学意义(P>0.05)。患者术后6个月、术后1年AOFAS、ATRS、Kaikkonen评分均高于术前,且术后1年高于术后6个月,差异均有统计学意义(P<0.05)。结论多孔金属板辅助下微创治疗急性闭合性跟腱断裂临床疗效可靠,值得临床推广。
Objective To explore the clinical effect of minimally invasive Achilles tendon repair assisted by porous metal plates for acute closed Achilles tendon rupture.Methods This study was a retrospective study including 19 patients who visited our hospital for minimally invasive Achilles tendon repair assisted by porous metal plates from March 1,2016 to December 31,2016.The length of surgical incision,operation time,intraoperative blood loss,postoperative complication,number of punctures,visual analogue scale(VAS)for pain,American Orthopedic Foot and Ankle Society Score(AOFAS)scores,Achilles tendon Total Rupture Score(ATRS),Kaikkonen score were included as comparators.Results The mean length of surgical incision,mean operation time,mean intraoperative blood loss,and mean number of punctures in the patients were(2.61±0.23)cm,(41.42±4.93)min,(13.22±2.07)mL,7(6,8),respectively.There was no postoperative complication in the patients.The VAS score of patients at day 0 was comparable to those at 1 day and 7 days postoperatively(P>0.05).The scores of AOFAS,ATRS,and Kaikkonen of patients at 6 months and 1 year postoperatively were significantly higher those at day 0,which at 1 year postoperatively were more pronounced than those at 6 month postoperatively.Conclusion The minimally invasive Achilles tendon repair assisted by porous metal plates for acute closed Achilles tendon rupture is reliable,and it is worthy of clinical promotion.
