摘要
目的甲状腺癌(TC)术后应用加味生脉散的治疗效果。方法选取2018年1月至2022年12月郑州市第一人民医院收治的60例TC术后患者,按随机数字表法分组,各30例。对照组接受常规西药治疗,观察组基于对照组加用加味生脉散,连续治疗3个月。统计两组临床疗效、中医证型评分、PI3K-AKT信号通路、甲状腺激素、不良反应。结果观察组治疗3个月后总有效率较对照组高(P<0.05);治疗1、3个月后观察组气短懒言、口咽干燥、疲倦乏力、五心烦热积分均低于对照组(P<0.05);治疗1、3个月后观察组促甲状腺激素、甲状腺球蛋白抗体、甲状腺过氧化物酶抗体水平低于对照组,游离三碘甲状腺原氨酸、游离甲状腺素水平及外周血单个核细胞PI3Kp110、AKT1 mRNA、蛋白均高于对照组(P<0.05)。结论TC术后应用加味生脉散效果确切,有助于改善中医证型及甲状腺功能,其作用机制可能与调控PI3K-AKT信号通路,促进细胞凋亡有关。
Objective To analyze the efficacy of using Jiawei Shengmai San for postoperative treatment of thyroid cancer(TC).Methods Sixty cases postoperative TC patients from Zhengzhou First People’s Hospital from January 2018 to December 2022 were selected and randomly divided into groups using a number table method,with 30 patients in each group.The control group was treated with conventional western medicine,while the observation group was treated with Jiawei Shengmai San for 3 months.The clinical efficacy,traditional Chinese medicine syndrome score,PI3K-AKT signaling pathway,thyroid hormones,and adverse reactions of the two groups were calculated.Results The total effective rate of the observation group was higher than that of the control group after 3 months of treatment(P<0.05).After 1 month and 3 months of treatment,the scores of shortness of breath and unwillingness to speak,dry mouth and pharynx,fatigue and lack of strength,and sphoria with feverish sensation in chest,palms and soles of the observation group were lower than those of the control group(P<0.05).After 1 month and 3 months of treatment,the levels of thyroid stimulating hormone,thyroglobulin antibodies and thyroid peroxidase antibody in the observation group were lower than those in the control group,and the levels of free triiodothyronine and free thyroxine,PI3Kp110 and AKT1 mRNA and protein in peripheral blood mononuclear cells were higher than those in the control group(P<0.05).Conclusion The application of Jiawei Shengmai San after TC surgery has a definite effect and helps to improve traditional Chinese medicine syndrome types and thyroid function.Its mechanism of action may regulate the PI3K-AKT signaling pathway and promote cell apoptosis.
