摘要
目的探究球囊子宫支架联合宫腔球囊扩张术防治宫腔粘连分离术后再粘连效果。方法取2021年1月至2023年5月在临沂市妇幼保健院妇科住院并行宫腔镜下宫腔粘连分离术的90例中重度宫腔镜粘连患者为研究对象。按患者术中辅助治疗方案的不同分为3组;对照组25例、试验A组30例、试验B组35例;对照组行宫腔镜子宫内膜粘连松解术;试验A组在对照组的基础上置入子宫球囊支架,试验B组在试验A组基础上,待子宫球囊支架取出后,行3次宫腔球囊扩张术(间隔1周)。3个月后,对比3组患者月经改善情况及宫腔形态恢复情况。结果对照组月经改善情况最差,宫腔形态恢复最差,试验A组月经改善情况较好,宫腔形态恢复较好,试验B组月经改善情况最好,宫腔形态恢复最好(P<0.05)。结论球囊子宫支架联合宫腔球囊扩张术防治宫腔粘连分离术后再粘连效果临床价值高,值得推广。
Objective To investigate the effect of balloon uterine stent combined with uterine balloon dilatation to prevent and control re-adhesion after separation of uterine adhesions;Methods 90 patients with moderate-to-severe uterine adhesions who were hospitalized in the Department of Gynecology,Women and Children's Health Care Hospital of Linyi and underwent separation of uterine adhesions with hysteroscopy from January 2021 to May 2023 were taken as the subjects of the study.The patients were divided into three groups according to the different intraoperative adjuvant treatment programs;25 cases in the control group,30 cases in the experimental group A,and 35 cases in the experimental group B.The control group underwent hysteroscopic endometrial adhesion release;the experimental group A had a uterine balloon stent inserted on top of the control group,and the experimental group B underwent three times of uterine balloon dilatation at one-week intervals on the basis of the experimental group A,after the removal of the uterine balloon stent.After three months,the improvement of menstruation and the recovery of uterine cavity morphology were compared among the three groups.Results The control group had the worst improvement in menstruation and the worst recovery of uterine cavity morphology,the experimental group A had better menstruation and better recovery of uterine cavity morphology,and the experimental group B had the best menstruation and the best recovery of uterine cavity morphology(P<0.05).Conclusions The combination of balloon uterine stent and uterine balloon dilatation has high clinical value in preventing and treating the re-adhesion of the uterine cavity after separation of uterine cavity adhesions and is worthy of popularization.
