摘要
目的观察清肺化痰汤辅治重症肺炎痰热壅肺证的临床效果。方法选取2020年9月—2022年5月西南医科大学附属中医医院收治的重症肺炎痰热壅肺证患者78例作为研究对象,按照随机数字表法分为观察组(n=39)和对照组(n=39)。对照组在常规治疗基础上给予广谱抗菌药物治疗,观察组在对照组基础上给予清肺化痰汤口服,2组均治疗2周。比较2组临床疗效,治疗前后中医证候积分、炎性因子[降钙素原(PCT)、C反应蛋白(CRP)、白介素-6(IL-6)、可溶性细胞间黏附分子-1(sICAM-1)]、肺功能指标[用力肺活量(FVC)、第1秒用力呼气容积(FEV_(1))、每分钟最大通气量(MVV)、呼气流量峰值(PEF)、FEV_(1)/FVC]。结果观察组总有效率为94.87%,高于对照组的76.92%(χ2=5.186,P=0.023)。治疗2周后,2组发热、气促、咳嗽评分与血清PCT、CRP、IL-6、sICAM-1水平均低于治疗前,且观察组低于对照组(P<0.01);2组FVC、FEV_(1)、MVV、FEV_(1)/FVC、PEF均高于治疗前,且观察组高于对照组(P<0.01)。结论清肺化痰汤辅治重症肺炎痰热壅肺证效果显著,可明显改善患者临床症状,抑制炎性反应,促进肺功能恢复。
Objective To observe the clinical effect of Qingfei Huatan decoction in treating severe pneumonia with phlegm-heat obstructing lung syndrome.Methods Seventy-eight patients with severe pneumonia with phlegm-heat obstructing lung syndrome admitted to the Affiliated Traditional Chinese Medicine Hospital of Southwest Medical University from September 2020 to May 2022 were selected as the study objects,and were divided into observation group(n=39)and control group(n=39)according to random number table method.The control group was given broad-spectrum antibiotics on the basis of conventional treatment,and the observation group was given Qingfei Huatan decoction orally on the basis of control group.Both groups were treated for 2 weeks.The clinical efficacy,TCM syndrome score,inflammatory factors[PCT,CRP,IL-6,sICAM-1],pulmonary function indexes[FVC,FEV1,MVV,PEF,FEV1/FVC]before and after treatment of the two groups were compared.Results The total effective rate of the observation group was 94.87%,which was higher than 76.92%of the control group(χ2=5.186,P=0.023).After two weeks of treatment,fever,shortness of breath,cough scores and serum PCT,CRP,IL-6,sICAM-1 levels in two groups were lower than before treatment,and observation group were lower than those of control group(P<0.01).FVC,FEV1,MVV,FEV1/FVC and PEF in two groups were higher than before treatment,and observation group were higher than those of control group(P<0.01).Conclusion Qingfei Huatan decoction has a remarkable effect in treating severe pneumonia with phlegm-heat obstructing lung syndrome,which can obviously improve clinical symptoms,inhibit inflammatory response and promote the recovery of lung function.
