摘要
目的探讨腹腔镜右半结肠癌根治术中尾侧入路联合中间翻页式清扫的疗效。方法选取2020年1月—2022年10月南通市第二人民医院(南通大学附属康复医院)收治的108例腹腔镜右半结肠癌根治术患者为研究对象,按随机数表法分为观察组(n=54,给予尾侧入路联合中间翻页式清扫)、对照组(n=54,给予中间入路)。对比两组手术相关指标、术后恢复时间、术后并发症发生率、为期12个月的随访结局。结果观察组术中操作时间、围手术期失血量、淋巴结清扫数量优于对照组,差异有统计学意义(P均<0.05)。两组术后初次流食摄入时间、首次肛门排气时间、首次下床活动时间、住院时间,差异无统计学意义(P均>0.05)。两组并发症发生率比较,差异无统计学意义(P>0.05)。两组无瘤生存率和总生存率比较,差异无统计学意义(P均>0.05)。结论腹腔镜右半结肠癌根治术中尾侧入路联合中间翻页式清扫的预后效果、安全性、术后恢复质量与常规中间入路相当,但相对而言,其存在耗时短、操作便捷、淋巴结清扫数量多、出血少等应用优势。
Objective To observe and explore the effect of caudal lateral approach combined with intermediate flap clearance in laparoscopic radical surgery for right colon cancer.Methods 108 patients of laparoscopic radical surgery for right colon cancer in Nantong Second People's Hospital(Kangfu Hospital Affiliated to Nantong University)from January 2020 to October 2022 were selected,and they were divided into the observation group(n=54,with caudal ap⁃proach combined with intermediate flap clearance)and the control group(n=54,with intermediate approach)according to the method of random number table.The two groups were compared in terms of operation-related indexes,postop⁃erative recovery time,postoperative complication rate,and follow-up outcomes over a period of 12 months.Results In⁃traoperative operation time,perioperative blood loss,and the number of lymph node dissection in the observation group were better than those in the control group,the differences were statistically significant(all P<0.05).The time of the first postoperative fluid intake,the time of the first anal defecation,the time of the first time to get out of bed,and the length of hospitalization in both group,the difference was not statistically significant(all P>0.05).The postopera⁃tive complications between two groups,the difference was not statistically significant(P>0.05).There was no statisti⁃cally significant difference in the postoperative period of 12 months dynamic follow-up,tumor-free survival rate,over⁃all survival rate between the two groups(all P>0.05).Conclusion The prognostic effect,safety,and quality of postoperative recovery of the caudal approach combined with intermediate flap clearance in laparoscopic radical surgery for right colon cancer are comparable to that of the conventional intermediate approach,but comparatively speaking,there are advantages of application such as shorter time-consuming,convenient operation,more quantity of lymph node dis⁃section,and less bleeding.
