小青龙汤联合麻龙平喘汤加减对儿童咳嗽变异性哮喘气道高反应性及免疫功能的影响

Effects of modified Xiaoqinglong decoction combined with Malong Pingchuan decoction on airway hyperreactivity and immune function in children with cough variant asthma

摘要 目的分析小青龙汤合麻龙平喘汤加减对儿童咳嗽变异性哮喘气道高反应性及免疫功能的影响。方法选取2022年1月至2022年12月于南阳市中医院收治的80例咳嗽变异性哮喘患儿,随机分为对照组及观察组(n=40)。对照组患儿实施常规治疗,观察组患儿在常规治疗基础上给予小青龙汤合麻龙平喘汤加减治疗。观察两组患儿的临床疗效、肺功能指标、炎症指标、免疫功能、气道高反应性、安全性及复发率。结果观察组临床总有效率(90.00%)显著高于对照组(72.50%)(P<0.05)。治疗后两组患儿用力肺活量(FVC)、第1秒用力呼气量(FEV1)及最大呼气峰流速(PEF)水平均升高,且观察组显著高于对照组(P<0.05)。治疗后两组患儿白细胞介素-8(IL-8)及嗜酸性粒细胞(EOS)水平下降,白细胞介素-10(IL-10)水平升高;且观察组IL-8及EOS水平显著低于对照组(P<0.001),IL-10水平显著高于对照组(P<0.001)。治疗后两组患儿CD4^(+)及CD4^(+)/CD8^(+)水平下降,CD8^(+)水平上升;且观察组CD4^(+)及CD4^(+)/CD8^(+)水平显著低于对照组(P<0.001),CD8^(+)水平显著高于对照组(P<0.001)。治疗后两组患儿FeNO下降,PD20上升;且观察组FeNO显著低于对照组(P<0.001),PD20显著高于对照组(P<0.05)。对照组共发生不良反应3例,总不良反应发生率为7.50%;观察组共发生不良反应4例,总不良反应发生率为10.00%;两组不良反应发生情况无显著差异(P>0.05)。随访12周发现观察组患儿复发率16.67%(6/36)显著低于对照组患儿复发率41.38%(12/29)(P<0.05)。结论小青龙汤合麻龙平喘汤加减对儿童咳嗽变异性哮喘具有较好的临床疗效,可有效改善患儿气道高反应性,调节免疫功能,安全性较高。 Objective To analyze the effect of modified Xiaoqinglong decoction combined with Malong Pingchuan decoc⁃tion on airway hyperreactivity and immune function of children with cough variant asthma.Methods A total of 80 chil⁃dren with cough variant asthma admitted to Nanyang Hospital of Traditional Chinese Medicine from January 2022 to De⁃cember 2022 were randomly divided into control group and observation group(n=40).The control group received rou⁃tine treatment,and the observation group received modified Xiaoqinglong decoction combined with Malong Pingchuan de⁃coction on the basis of routine treatment.The clinical efficacy,pulmonary function index,inflammatory index,immune function,airway hyperresponsiveness,safety,and recurrence rate in the two groups were observed.Results The total effective rate of observation group(90.00%)was significantly higher than that of control group(72.50%)(P<0.05).After treatment,the levels of FVC,FEV1,and PEF in both groups were increased,and these indexes in observation group were significantly higher than those in the control group(P<0.05).After treatment,the levels of IL-8 and EOS decreased,while the level of IL-10 increased.The levels of IL-8 and EOS in the observation group were significantly lower than those in the control group(P<0.001),and the level of IL-10 was significantly higher than that in the control group(P<0.001).After treatment,the levels of CD4^(+)and CD4^(+)/CD8^(+)decreased,while the level of CD8^(+)increased.CD4^(+)and CD4^(+)/CD8^(+)levels in observation group were significantly lower than those in con⁃trol group(P<0.001),and CD8^(+)level was significantly higher than that in control group(P<0.001).FeNO de⁃creased and PD20 increased in both groups after treatment.FeNO in the observation group was significantly lower than that in the control group(P<0.001),and PD20 was significantly higher than that in the control group(P<0.05).In the control group,there were 3 cases of adverse reactions,the total incidence of adverse reactions was 7.50%.In the observ
