摘要
目的:研究孕早期白细胞计数(WBC)、中性粒细胞百分比、淋巴细胞百分比、中性粒细胞/淋巴细胞比值(NLR)、胆红素等与妊娠期糖尿病(GDM)的相关性,为GDM的早期筛查提供新的思路、临床检测指标。方法:回顾分析2022年6月至2022年12月甘肃省妇幼保健院收治的87例GDM患者的临床资料,选取同期87例正常孕妇为对照组。比较GDM组与正常孕妇组的孕早期WBC计数、中性粒细胞百分比、淋巴细胞百分比、NLR、胆红素、孕中期空腹血糖指标。结果:二元logistic回归分析结果显示,孕早期WBC、胆红素对GDM患病有显著影响(OR=1.468,OR=1.185,P均<0.05)。WBC联合胆红素对GDM的早期预测价值大于单独预测价值。结论:WBC计数、胆红素可作为GDM孕早期筛查、监测指标。
Objective:To study the correlation between white blood cell count(WBC),neutrophil percentage,lymphocyte percentage,neutrophil/lymphocyte ratio(NLR),bilirubin,etc.and gestational diabetes mellitus(GDM)in the early stage of pregnancy,and to provide a new way of thinking and a clinical test index for the early screening of GDM.Methods:The clinical data of 87 GDM patients admitted to Gansu Maternal and Child Health Hospital from June 2022 to December 2022 were reviewed and analyzed,and 87 normal pregnant women in the same period were selected as the control group.The WBC count,neutrophil percentage,lymphocyte percentage,NLR,bilirubin,and fasting blood glucose in the second trimester were compared between the GDM group and the normal pregnant woman group.Results:Binary logistic regression analysis showed that WBC and bilirubin in the first trimester had a significant effect on the prevalence of GDM(OR=1.468,OR=1.185,both P<0.05).WBC combined with bilirubin had a greater predictive value than that of bilirubin alone in the early prediction of GDM.Conclusion:WBC count and bilirubin can be used as indicators for screening and monitoring of GDM in early pregnancy.
