摘要
目的探讨CT三维重建影像特征在纯磨玻璃结节(pGGN)病灶生物学行为评估中的应用价值。方法回顾性选取2019年3月至2021年9月于萍乡市人民医院行CT三维重建且影像征象为pGGN的93例患者作为研究对象,根据其病理结果不同分为良性组(n=40)与恶性组(n=53)。观察两组CT三维重建影像特征,分析其不同病理特征的差异。结果恶性组的病灶直径大于良性组,病灶边界光滑、形态不规则、呈现毛刺征、血管集束征、分叶征、支气管充气征、胸膜凹陷征、空泡征的占比均高于良性组,差异有统计学意义(P<0.05);ROC曲线分析结果显示,CT各征象(边界、直径、形态、毛刺征、血管集束征、分叶征、支气管充气征、胸膜凹陷征、空泡征)诊断pGGN恶性病变的价值均较好,其中边界光滑、分叶征、胸膜凹陷征的AUC较高。结论利用CT三维重建影像学征象可为临床提供判断pGGN良恶性病变的依据,其中以病灶的边界表现、分叶征和胸膜凹陷征最具有诊断pGGN恶性病变的价值。
Objective To evaluate the application of three-dimensional CT reconstruction imaging characteristics in evaluating biological behav-iors of pure ground-glass nodules.Methods A total of 93 patients with pGGN who underwent three-dimensional CT reconstruction reconstruction in Pingxiang People's Hospital from March 2019 to September 2021 were retrospectively selected as the study subjects,and they were divided into the benign group(n=40)and the malignant group(n=53)according to th in diageir pathological findings.The three-dimensional CT reconstruction image features of the two groups were observed and the differences of different pathological features were analyzed.Results The diameter of lesions in the malignant group was larger than that in the benign group,and the proportion of lesions with smooth boundary,irregular shape,burr sign,vascular cluster sign,lobular sign,air bronchogram sign,pleural depression sign and vacuole sign were higher than those in the benign group,the differences were statistically significant(P<0.05).ROC curve analysis results showed that all CT signs(boundary,diameter,shape,burr sign,vascular cluster sign,lobular sign,air bronchial sign,pleural depression sign,vacuole sign)were of good value in diagnosing pGGN malignant lesions,and the AUC of smooth boundary,lobular sign,pleural depression sign was higher.Conclusion Three-dimensional CT reconstruction can provide the basis for clinical diagnosis of benign and malignant pGGN lesions,among which the boundary manifestations,lobular signs and pleural depression signs are the most valuable in the diagnosis of pGGN malignant lesions.
