宫颈癌术后放疗患者下肢静脉彩色多普勒超声云雾状回声与下肢静脉血栓形成风险的相关性

摘要 目的探讨宫颈癌术后放疗患者下肢静脉彩色多普勒超声云雾状回声与下肢静脉血栓形成风险的相关性。方法选取2018年1月至2022年12月新余市人民医院收治的60例宫颈癌术后放射治疗患者作为研究对象,根据放射治疗是否伴有下肢静脉血栓形成分为研究组(伴有下肢静脉血栓形成)与对照组(未伴有下肢静脉血栓形成),每组30例。两组均于术后放疗初期接受彩色多普勒超声检查,比较两组凝血指标、下肢静脉血栓形成风险分级、云雾状回声出现率,Logistic回归分析下肢静脉血栓形成的影响因素。结果研究组凝血酶时间(TT)、凝血酶原时间(PT)均长于对照组,纤维蛋白原(FIB)和D-二聚体(D-D)水平均高于对照组,差异有统计学意义(P<0.05)。研究组下肢静脉血栓形成风险3、4级占比均高于对照组,差异有统计学意义(P<0.05)。Logistic回归分析结果显示,TT、PT、FIB、D-D异常及出现云雾状回声是下肢静脉血栓形成的危险因素(P<0.05)。结论宫颈癌术后放疗患者下肢静脉彩色多普勒超声云雾状回声是下肢静脉血栓形成的影响因素,与凝血指标的一致性良好,能预测宫颈癌术后放疗下肢静脉血栓形成的风险,为预见性治疗的开展提供有力依据。 Objective To investigate the correlation between the cloud echo of lower extremity venous ultrasound and the risk of lower extremity venous thrombosis in patients with postoperative radiotherapy for cervical cancer.Methods 60 patients with cervical cancer treated with postopera-tive radiotherapy in Xinyu People's Hospital from January 2018 to December 2022 were selected as the study subjects,they were divided into the study group(accompanied by lower limb venous thrombosis)and the control group(not accompanied by lower limb venous thrombosis)according to whether radiotherapy was accompanied by lower limb venous thrombosis,with 30 cases in each group.Both groups received color Doppler ultraso-nography at the early stage of postoperative radiotherapy,the coagulation index,risk grade of lower limb venous thrombosis and cloud echo occur-rence rate were compared between the two groups,and the influencing factors of lower limb venous thrombosis were analyzed by Logistic regres-sion.Results The thrombin time(TT),prothrombin time(PT)in the study group were longer than those in the control group,and the levels of fibrino-gen(FIB)and D-dimer(D-D)were higher than those in the control group,and the differences were statistically significant(P<0.05).The proportion of grade 3 and grade 4 thrombosis risk in the study group were higher than those in the control group,and the differences were statistically significant(P<0.05).Logistic regression analysis showed that abnormal TT,PT,FIB,D-D and cloud echo were the risk factors of lower limb venous thrombosis(P<0.05).Conclusion The cloud echo of lower limb venous ultrasound is the influence of lower limb venous thrombosis in patients with postopera-tive radiotherapy for cervical cancer,which has a good consistency with coagulation indexes,and can be used to predict the risk of lower limb venous thrombosis after postoperative radiotherapy for cervical cancer,providing a strong basis for the development of predictive therapy.
