摘要
目的评价三维步态动作捕捉与训练系统对脑卒中患者偏瘫步态康复效果的影响。方法共有脑卒中后偏瘫患者60例,随机分为对照组、观察组,均30例。两组患者均给予常规康复训练,观察组在常规康复训练基础上进行虚拟现实步态训练。在治疗前、连续治疗6周后由同一位治疗师给予两组患者采用三维步态分析系统检测并比较两组患者步态时间参数、时相参数、关节活动角度等参数进行评估,并采用Holden步行评定量表,Barthel指数评定表及下肢Fugl-Meyer运动功能量表对患者日常生活能力、步行功能康复效果进行评估。结果两组患者的一般资料(性别、年龄、病变性质)及Holden步行等级、FMA-L评分、Barthel评分与三维步态参数均差异无统计学意义(P>0.05)。而在干预后两组在三维步态参数(时间参数、时相参数、关节活动角度)、FMA-L评分、Holden步行等级、Barthel评分均较之前有所提高(P<0.05),而与治疗前比较,步行周期、双支撑相(%)、健侧支撑相时间(%)、患侧支撑相时间(%)均明显缩短(P<0.05),但同时健侧摆动相时间(%)、患侧摆动相时间(%)均与之相反(P<0.05);与对照组比较,观察组在步幅、步频、步速、健侧支撑相(%)、健侧摆动相(%)、双支撑相(%)方面均明显更优,差异有统计学意义(P<0.05)。结论三维步态动作捕捉系统用于脑卒中患者,能够有效量化患者步态参数,有利于为康复训练提供依据;其步态训练系统能够有效改善脑卒中偏瘫步态康复效果。
Objective To evaluate the effect of three-dimensional gait motion capture and training system on the rehabilitation effect of hemiplegic gait in stroke patients.Methods A total of 60 patients with post-stroke hemiplegia were randomly divided into the routine group and the observation group,with 30 cases in each group.Both groups were given routine rehabilitation training,and the observation group received virtual reality gait training on the basis of routine rehabilitation training.Before treatment and after 6 weeks of continuous treatment,the same therapist gave the patients of the two groups a three-dimensional gait analysis system to detect and compare the gait time parameters,phase parameters,joint motion angle and other parameters of the two groups.The walking rating scale,the Barthel index rating scale and the lower limb Fugl-Meyer motor function scale were used to evaluate the patients′daily living ability and the rehabilitation effect of walking function.Results There were no significant differences in general information(gender,age,nature of lesions),Holden walking scale,Fugl-Meyer assessment(FMA-L)score,Barthel score or three-dimensional gait parameters between the two groups(P>0.05).After the intervention,the three-dimensional gait parameters(time parameter,phase parameter,joint motion angle),FMA-L score,Holden′s walk grade and Barthel score were all improved in the two groups(P<0.05).Compared with those the previous comparison,the walking cycle,double support phase(%),unaffected side support phase time(%)and ipsilateral support phase time(%)were significantly shortened(P<0.05),but at the same time the unaffected side swing phase time(%)and the swing phase time(%)of the affected side were opposite to that(P<0.05).Compared with the control group,the observation group showed the improvement of stride length,stride frequency,stride speed,support phase(%)and swing phase(%)of the healthy side,double support phase(%),and there were significant differences(P<0.05).Conclusion The three-dimensional gait mot
