摘要
目的 探讨和分析高频超声在新生儿呼吸窘迫综合(NRDS)与湿肺中的诊断价值。方法 该回顾性研究纳入2022年1月—2023年6月在我院住院治疗的40例新生儿,20例NRDS新生儿纳入至NRDS组,20例湿肺患儿纳入至湿肺组,应用高频超声检查对所有新生儿进行肺部检查,比较两组新生儿肺部超声评分、肺部超声征象,以临床诊断作为金标准,评价高频超声对NRDS与湿肺的鉴别诊断价值。结果 NRDS组新生儿肺部超声评分高于湿肺组(P <0.05)。NRDS新生儿病情越严重者则肺部超声评分越高(P <0.05)。NRDS组新生儿融合B线、肺泡-间质综合征、A线消失、胸膜线异常及肺实变等肺部超声征象检出率均高于湿肺组(P <0.05)。高频超声在NRDS及新生儿湿肺中的敏感度、准确度、特异度、阳性预测值、阴性预测值差异均无统计学意义(P> 0.05)。结论 高频超声在新生儿NRDS与湿肺中的诊断效能均较高,能够对NRDS与湿肺进行鉴别,还能够判断NRDS严重程度。
Objective To analyze the diagnostic value of high-frequency ultrasound in neonatal respiratory distress synthesis(NRDS)and wet lung.Methods This retrospective study included 40 neonates hospitalized in our hospital from January 2022 to June 2023,20 NRDS neonates were included in the NRDS group,and 20 children with wet lung were included in the wet lung group.All neonates were examined by highfrequency ultrasound,and the pulmonary ultrasound scores and pulmonary ultrasound signs of the two groups were compared.Clinical diagnosis was used as the gold standard to evaluate the value of high frequency ultrasound in the differential diagnosis of NRDS and wet lung.Results The ultrasound score of neonatal lung in NRDS group was higher than that in wet lung group(P<0.05).The more severe the condition of NRDS,the higher the pulmonary ultrasound score(P<0.05).The detection rates of pulmonary ultrasound signs such as fusion line B,alveolar-interstitial syndrome,disappearance of line A,abnormal pleural line and lung consolidation in NRDS group were higher than those in wet lung group(P<0.05).There were no significant differences in sensitivity,accuracy,specificity,positive predictive value and negative predictive value of high frequency ultrasound in NRDS and neonatal wet lung(P>0.05).Conclusions High frequency ultrasound is effective in the diagnosis of neonatal NRDS and wet lung.It can distinguish NRDS from wet lung and judge the severity of NRDS.
